社区精神分裂症患者服药依从性现况调查

Prevalence of Medicine compliance of patients with schizophrenia in community:a Cross-section Survey

目的探讨社区精神分裂症患者服药依从性情况。方法采用分层整群随机抽样的方法,按照城市和农村划分分别抽取4个街道或乡镇,采用面对面的调查方式对155例患者进行调查。结果社区精神分裂症患者中,服药完全依从者59例(38.1%),服药部分依从54例(34.8%),服药不依从42例(27.1%);不同地区服药部分依从或不依从的构成差异无统计学意义(χ~2=0.79,P=0.37)。患者服药部分依从和不依从主要原因为他们认为病情稳定就可以减药或停药、抗精神病药物不良反应和认为自己没病,不需要再服药等。结论社区精神分裂症患者中服药部分依从或不依从比例仍然较高,需要加强患者及家属的健康教育,联合社区进行综合干预和管理、选用副反应小的药物是提高患者服药依从性的重要手段。

Objective To evaluate the medication compliance of patients in community with schizophrenia.Methods A randomized stratifiedcluster sampling method was used,and extracted4sub-districts from city and countryside respectively,and in total155patients with schizophreniain the community were sampled and completed questionnaire by face to face views.Results Among the community schizophrenic patients,59cases(38.1%)were with full medication compliance,54cases(34.8%)with partly medication compliance,42cases(27.1%)with medication non-compliance.There is no statistically significance in proportion of partly medication compliance or non-compliance between urban and rural regions.Themain reasons for patients with part-compliance and non-compliance,were patients though illness was stable and can reduce or stop taking medicine;worried about anti-psychiatric side effects;and considered they had no disease,and no need to take medication.Conclusion In community,the proportionsof patients with schizophrenia in partly medication compliance and non-compliance were still high,need to strengthen the health educationfor patients and their relatives,combined with community comprehensive intervention and management,and selected medicine of small side effectwere the effective means for improving community schizophrenia patients’medication compliance.
