微型钢板与克氏针内固定治疗掌骨骨折的临床疗效对比研究

Comparative research of clinical effects by mini-plate and Kirschner wire internal fixation in the treatment of metacarpal fracture

目的对比分析微型钢板与克氏针内固定治疗掌骨骨折的临床疗效。方法 62例掌骨骨折患者按照治疗方法不同分为A组与B组,各31例。A组采用微型钢板内固定,B组采用克氏针内固定,比较两组患者功能优良率、手术时间、住院时间、并发症发生率、骨折愈合时间及外观满意度。结果 A组功能优良率为90.32%,明显高于B组的64.52%,差异具有统计学意义(P<0.05)。A组手术时间与B组比较更长,骨折愈合时间与B组比较更短,差异均具有统计学意义(P<0.05);A组住院时间与B组比较稍长,但差异无统计学意义(P>0.05)。两组并发症主要表现为术后感染、肌腱黏连、延迟愈合等,A组术后并发症发生率为6.46%(2/31),B组并发症发生率为22.58%(7/31),A组术后并发症发生率与B组比较更低,差异具有统计学意义(P<0.05)。A组外观满意度为93.55%,明显高于B组的70.97%,差异具有统计学意义(P<0.05)。结论微型钢板治疗掌骨骨折患者有显著疗效,患者术后可快速恢复,安全性高,患者外观满意度更高,可作为治疗掌骨骨折的一种有效方法在临床推广应用。

Objective To compare and analyze clinical effects by mini-plate and Kirschner wire internal fixation in the treatment of metacarpal fracture. Methods A total of 62 patients with metacarpal fracture were divided by different treatment measures into group A and group B, with 31 cases in each group. Group A received mini-plate internal fixation, and group B received Kirschner wire internal fixation. Comparison was made on function good rate, operation time, hospital stay time, incidence of complications, fracture healing time and appearance satisfaction between the two groups. Results Group A had obviously higher function good rateas 90.32% than 64.52% in group B, and the difference had statistical significance (P<0.05). Group A had longeroperation time and shorter fracture healing time than group B, and their differences all had statistical significance(P<0.05). Group A had slightly longer hospital stay time than group B, while their difference had no statisticalsignificance (P>0.05). Main complications in the two groups included postoperative infection, tendon adhesion anddelayed union. Group A had incidence of postoperative complications as 6.46% (2/31), which was 22.58% (7/31)in group B. Group A had lower incidence of postoperative complications than group B, and their difference hadstatistical significance (P<0.05). Group A had much higher appearance satisfaction as 93.55% than 70.97% in group B, and the difference had statistical significance (P<0.05). Conclusion Mini-plate shows excellent effect in treating metacarpal fracture, along with quick postoperative recovery, high safety and appearance satisfaction in patients. As an effective method in treating metacarpal fracture, it is worth clinical promotion and application.
