预见性护理预防全髋关节置换术下肢深静脉血栓形成的临床效果观察

Observation on clinical effect of predictive nursing in prevention of lower extremity deep venous thrombosis after total hip replacement arthroplasty

目的探究在预防全髋关节置换术下肢深静脉血栓形成中实行预见性护理方法的临床效果。方法 78例行全髋关节置换术的患者作为研究对象,随机分为观察组和对照组,各39例。对照组行常规性护理方法 ,观察组采取预见性护理方法。对比两组临床效果。结果两组患者护理前血流速度比较,差异无统计学意义(t=0.1317,P>0.05);护理2、4周后,观察组均快于对照组,差异均具有统计学意义(t=7.0840、6.4115,P<0.05)。观察组血栓发生率为5.1%、发生时间为(9.3±0.5)d、住院时间为(25.6±2.7)d,均优于对照组的血栓发生率23.1%、发生时间(5.4±0.3)d、住院时间(36.4±3.4)d,差异均具有统计学意义(P<0.05)。结论将预见性护理方法应用于预防全髋关节置换术中,能够最大限度的防止患者发生下肢深静脉血栓,加快患者的下肢静脉血流速度,具有较强的临床推广价值。

Objective To explore the clinical effect of predictive nursing in lower extremity deep venous thrombosis after total hip replacement arthroplasty. Methods A total of 78 patients with total hip replacement arthroplasty as study subjects were randomly divided into observation group and control group, with 39 cases in each group. The control group received conventional nursing method and the observation group received predictive nursing method. Clinical effect was compared in two groups. Results There was no statistically significant difference in blood flow velocity before nursing in two groups(t=0.1317, P>0.05). After 2 and 4 weeks of nursing, the observation group had faster blood flow velocity than the control group, and the difference had statistical significance(t=7.0840, 6.4115, P<0.05). The observation group had incidence of thrombosis as 5.1%, occurrence time as(9.3±0.5)d, hospital stay time as(25.6±2.7)d, which were all better than 23.1%,(5.4±0.3)d and(36.4±3.4)d in the control group. Their differences had statistical significance(P<0.05). Conclusion Application of redictive nursing in total hip replacement arthroplasty, can maximumly prevent the occurrence of lower extremity deep venous thrombosis, quicken blood flow velocity in lower limb venous. So this method contains high clinical promotion value.
