超声联合宫腔镜诊断子宫内膜异常回声100例分析

Analysis of ultrasound combined with hysteroscope in the diagnosis of 100 cases of abnormal endometrial echo

目的研究并探讨超声联合宫腔镜诊断子宫内膜异常回声的价值。方法 100例子宫内膜异常回声患者,采取回顾性研究的方法对患者的临床资料进行分析,所有患者均接受腹部超声检查、阴道超声检查、宫腔镜病理检查,以宫腔镜病理检查结果作为诊断金标准,计算腹部超声检查、阴道超声检查对子宫内膜异常回声的检出率及其对各类病变的诊断符合率。结果宫腔镜病理检查结果显示,100例子宫内膜异常回声患者中包括40例子宫内膜息肉患者、25例子宫内膜增生症患者、15例子宫黏膜下肌瘤患者、4例子宫内膜腺肌瘤型息肉患者、1例子宫内膜癌患者、15例宫腔胚胎组织残留患者。腹部超声检查、阴道超声检查对子宫内膜异常回声的诊断符合率分别为88.00%、96.00%,比较差异具有统计学意义(P<0.05)。腹部超声检查出现的12例漏诊包括子宫内膜息肉5例、子宫黏膜下肌瘤2例、宫腔胚胎组织残留5例,阴道超声检查出现的4例漏诊包括子宫内膜息肉1例、宫腔胚胎组织残留3例。结论阴道超声检查对子宫内膜异常回声的诊断准确性相对于腹部超声检查更高,临床上针对子宫内膜异常回声患者进行诊断的最佳方法为阴道超声检查联合宫腔镜检查。

Objective To study and explore the value of ultrasound combined with hysteroscope in thediagnosis of abnormal endometrial echo.Methods Clinical data of100abnormal endometrial echo patients wereretrospectively analyzed.All patients received abdominal ultrasound examination,vaginal ultrasound examinationand pathological examination by hysteroscope.Pathological examination outcome by hysteroscope was taken asthe diagnostic gold standard to calculated detection rate of abnormal endometrial echo by abdominal ultrasoundexamination,vaginal ultrasound examination,and diagnostic accordance ratence rate of all kinds of pathologicalchanges.Results According to pathologic examination outcome by hysteroscope,there were40endometrialpolyp cases,25endometrial hyperplasia cases,15submucous myoma of uterus cases,4endometrial gland fibroidspolyp cases,1endometrial cancer case,15intrauterine embryo tissue residues cases in100abnormal endometrialecho cases.Abdominal ultrasound examination,vaginal ultrasound examination had diagnostic accordance rateof abnormal endometrial echo respectively as88.00%,96.00%,and their differences had statistical significance(P<0.05).There were12missed diagnostic cases by abdominal ultrasound examination,including5endometrialpolyp cases,2submucous myoma of uterus cases,5intrauterine embryo tissue residues cases.There were4missed diagnostic cases by vaginal ultrasound examination,including1endometrial polyp case and3intrauterineembryo tissue residues cases.Conclusion Vaginal ultrasound examination has higher diagnostic accuracy thanabdominal ultrasound examination for abnormal endometrial echo,and the best diagnostic method for abnormalendometrial echo is combination of vaginal ultrasound examination and hysteroscope examination.
