探析关节滑液血管活性肠肽水平与膝前交叉韧带损伤后关节软骨退变的相关性

Exploration on correlation between synovial fluid vasoactive intestinal peptide levels and articular cartilage degeneration after anterior cruciate ligament injury

目的研究并探讨关节滑液血管活性肠肽(VIP)水平与膝前交叉韧带(ACL)损伤后关节软骨退变的相关性。方法 80例膝ACL损伤患者,根据ACL损伤后是否伴有关节软骨退变分为退变组(49例)、无退变组(31例),比较两组患者关节滑液VIP水平。根据关节软骨退变患者的关节软骨损伤情况将其分为轻度退变组(28例)、重度退变组(21例),比较两组患者关节滑液VIP水平,并计算关节滑液VIP水平与膝ACL损伤后关节软骨退变的相关性。结果退变组患者的关节滑液VIP水平为(151.64±78.91)pg/ml,明显低于无退变组患者的(474.29±183.79)pg/ml,差异有统计学意义(P<0.05)。轻度退变组患者的关节滑液VIP水平为(218.29±85.96)pg/ml,明显高于重度退变组患者的(69.37±39.24)pg/ml,差异有统计学意义(P<0.05)。关节滑液VIP水平与膝ACL损伤后关节软骨退变呈负相关(r=-0.745,P=0.001<0.05)。结论关节滑液VIP可能参与到膝ACL损伤后关节软骨退变的发生、发展过程中,在一定程度上可反映关节软骨退变的严重程度,可作为判断关节软骨损伤程度的重要标志物。

Objective To study the correlation between synovial fluid vasoactive intestinal peptide(VIP)levels and articular cartilage degeneration after anterior cruciate ligament(ACL)injury.Methods A totalof80ACL injury patients were divided by whether complicated with articular cartilage degeneration after ACLinjury into degeneration group(49cases)and non degeneration group(31cases).Synovial fluid VIP levels werecompared in two groups.The degeneration group was divided by different articular cartilage injury into milddegeneration group(28cases)and severe degeneration group(21cases).Synovial fluid VIP levels were comparedin two groups and correlation between synovial fluid VIP levels and articular cartilage degeneration after ACLinjury.Results The degeneration group had obviously lower synovial fluid VIP levels as(151.64±78.91)pg/mlthan(474.29±183.79)pg/ml in the non degeneration group,and the difference had statistical significance(P<0.05).The mild degeneration group had obviously higher synovial fluid VIP levels as(218.29±85.96)pg/ml than(69.37±39.24)pg/ml in the severe degeneration group,and the difference had statistical significance(P<0.05).Synovial fluid VIP levels and articular cartilage degeneration after ACL injury were negatively correlated(r=-0.745,P=0.001<0.05).Conclusion Synovial fluid VIP may participate in occurrence,development process of articularcartilage degeneration after ACL injury.It can reflect the severity of the articular cartilage degeneration to acertain extent,which can be used as an important marker of articular cartilage injury degree.
