剖宫产后再次妊娠三种流产方法比较

Comparison of three abortion methods of subsequent pregnancy after cesarean section

目的比较剖宫产后再次妊娠不同流产方法的优劣。方法 69例剖宫产后再次妊娠要求流产患者作为研究对象,遵循患者意愿,根据流产方式的不同分为A组(行床边B超监测下无痛人工流产)、B组(行药物流产)、C组(行传统无痛人工流产),每组23例。对比三组患者的流产结果、疼痛程度、术中出血量及手术时间。结果 A组(100.00%)和B组(91.30%)患者完全流产率均高于C组(60.87%),差异均有统计学意义(P<0.05);但A组和B组比较,差异无统计学意义(P>0.05)。A组和B组疼痛程度均优于C组,A组疼痛程度优于B组,差异均具有统计学意义(P<0.05)。A组患者术中出血量为(20.40±8.13)ml、手术时间为(42.01±12.29)s,C组患者术中出血量为(26.78±10.97)ml、手术时间为(81.36±18.60)s,A组患者术中出血量少于C组、手术时间短于C组,差异均具有统计学意义(P<0.05)。结论药物流产和B超监测下无痛人工流产均具有较好的临床效果,要结合患者的实际情况灵活运用。

Objective To compare the pros and cons of three abortion methods of subsequent pregnancy after cesarean section.Methods A total of69subsequent pregnancy after cesarean section patients withrequired abortion as study subjects were divided by different abortion methods into group A(received painlessartificial abortion by bedside B-mode ultrasonography monitoring),group B(received drug abortion)and group C(received traditional painless artificial abortion),with23cases in each group.Comparison were made on abortionoutcome,pain degree,intraoperative bleeding volume and operation time in three groups.Results Group A andgroup B had complete abortion rate as100.00%and91.30%,which were higher than60.87%in group C,and thedifference had statistical significance(P<0.05),while there was no statistically significant difference between groupA and group B(P>0.05).Group A and group B had better pain degree than group C,and group A had better paindegree than group B.Their difference had statistical significance(P<0.05).Group A had intraoperative bleedingvolume as(20.40±8.13)ml,operation time as(42.01±12.29)s,while group C had intraoperative bleedingvolume as(26.78±10.97)ml,operation time as(81.36±18.60)s.Group A had less intraoperative bleedingvolume than group C,and shorter operation time than group C.Their difference had statistical significance(P<0.05).Conclusion Both drug abortion and painless artificial abortion by bedside B-mode ultrasonography monitoringshow good clinical effect.Abortion methods should be flexibly used according to actual situation of patients.
