慢性心衰合并房颤患者血NT-proBNP水平及临床相关性研究

Clinical study on the relationship between serum NT-proBNP level and chronic heart failure patients complicated with atrial fibrillation

目的:探讨慢性心衰合并房颤患者血NT-proBNP水平及其临床相关性。方法:收治慢性心衰房颤或窦性心律患者各60例,无心衰和房颤患者60例为对照组。结果:慢性心衰患者,房颤组血NT-proBNP水平高于窦性心律组,且均高于对照组(P<0.05);且血NT-proBNP水平与LA、LVEDD呈正相关(P<0.01),与LVEF呈弱负相关(P<0.05);房颤组患者血NT-proBNP水平与HR呈正相关(P<0.01),与LA呈正相关(P<0.01)。结论:血NT-proBNP水平可以协助诊断心衰,且随着心肌重构及收缩功能减退而升高;房颤时血NT-proBNP水平进一步升高,血NT-proBNP水平可作为房颤患者心功能下降的预测因子。

Objective:To investigate the serum NT-proBNP level and its clinical relationship in patients with chronic heart failure complicated with atrial fibrillation.Methods:60 cases of chronic heart failure with atrial fibrillation or sinus rhythm were selected.60 patients without heart failure and atrial fibrillation were selected as control group.Results:In patients with chronic heart failure,the blood NT-proBNP level of atrial fibrillation group was higher than that of the sinus rhythm group,and were higher than that of the control group(P<0.05);the serum levels of NT-proBNP and LA,LVEDD hand positive correlation(P<0.01),showed a weak negative correlation with LVEF(P<0.05);there was a positive correlation between the level of serum NT-proBNP and HR in atrial fibrillation group(P<0.01),which was positively correlated with LA(P<0.01).Conclusion:The blood level of NT-proBNP can assist in the diagnosis of heart failure,and increased with myocardial remodeling and systolic dysfunction;the level of serum NT-proBNP was further increased in atrial fibrillation, and the serum NT-proBNP level could be used as a predictor of cardiac function decline in patients with atrial fibrillation.
