四肢创伤开放性合并粉碎性骨折患者的临床特点和手术治疗 被引量:13

摘要 目的:探讨四肢创伤开放性合并粉碎性骨折患者病情特点及采取手术方案治疗的效果。方法:收治四肢创伤开放性合并粉碎性骨折患者22例,共骨折71处,分别采用交锁髓内钉、外固定支架、钢板内固定治疗,回顾临床资料。结果:髓内钉内固定患者愈合率、邻近关节功能ROM优良率明显高于外固定支架、钢板内固定患者,差异具有统计学意义(P<0.05),畸形愈合和骨不连率低于外固定支架、钢板内固定患者。结论:四肢创伤开放性合并粉碎性骨折患者采用髓内钉内固定方案治疗,可促进邻近关节功能恢复,提高愈合率,降低骨不连和畸形愈合率。 Objective:To investigate the characteristics of patients with open and comminuted fractures of the extremities and the effect of surgical treatment.Methods:22patients with open and comminuted fractures of the extremities were selected,a total of71fractures.The patients were treated with interlocking intramedullary nail,external fixation and internal fixation with steel plate.We reviewed the clinical data.Results:The rate of healing rate and ROM of adjacent joint function in patients with intramedullary nailfixation was significantly higher than that in patients with external fixation and plate fixation(P<0.05).The rate of malunion andnonunion was lower than that of external fixation and plate fixation.Conclusion:The treatment of open comminuted fractures of the extremities with intramedullary nail fixation can promote the recovery of joint function and improve the healing rate,reduce the rate of nonunion and malunion.
