从肝肾脾胃论治特发性矮身材之我见

Idiopathic short stature treatment based on syndrome differentiation of liver, kidney, spleen and stomach

特发性矮身材是矮小症中最常见的类型,西医对该病的认识尚未达成一致;中医对该病的病因病机、辨证分型报道甚少。笔者依据长期的临床经验,结合中医理论和相关文献报道,认为该病的发病与肝肾脾胃关系密切,和肝益肾、调补脾胃当为其基本治法,宜将该病分为肝郁不舒、脾胃失和、肾阳不足、肝脾失调、肝肾亏虚和脾肾两虚六型,分别以柴胡疏肝散、健脾丸合木金丸、金匮肾气丸、逍遥散、六味地黄丸和附子理中丸为基础方剂,并举隅医案,证实从肝肾脾胃论治颇具良效,值得临床借鉴。

Idiopathic short stature is the most common type of dwarfism,of which there is no consensus regarding academic understanding of the disease in modern medicine.TCM doctors have reported very little in the aspect of the etiology,pathogenesis,syndrome differentiation and classification of this disease.The author,on the basis of long-term clinical experiences,combines TCM theory and related literature reports,and holds that the onset of this disease is closely related to liver,kidney,spleen and stomach,and the basic treatment should be regulating liver,kidney,spleen and stomach,and the disease should be divided into six types including stagnation of liver qi,disorder of spleen and stomach,insufficiency of kidney-yang,liver-spleen disharmony,deficiency of liver and kidney,asthenia of both spleen and kidney and Chaihu Shugan power,Jianpi Pill combined with Mujin pill,Jingui Shenqi pill,Xiaoyao power,Liuwei Dihuang pill and Fuzi Lizhong pill should be the basic formula respectively.The author also confirms that treatment from the liver,kidney,spleen and stomach differentiation has significant effect through illustrating examples,therefore,it is worthy of clinical reference.
