瘢痕子宫足月妊娠阴道分娩的临床观察

Clinical observation of full-term pregnancy vaginal delivery with scarred uterus

目的对瘢痕子宫再次足月妊娠的分娩方式进行探讨。方法 80例瘢痕子宫再次足月妊娠者,将其按照分娩方式不同分为阴道分娩组(选择阴道分娩)和剖宫产组(选择剖宫产),每组40例,对两组产妇的分娩出血量、分娩成功率、并发症和新生儿Apgar评分进行比较。结果阴道分娩组出血量(116.45±21.48)ml少于剖宫产组(220.42±21.46)ml,差异具有统计学意义(P<0.05)。阴道分娩组分娩成功率(80.00%)低于剖宫产组(100.00%),差异具有统计学意义(P<0.05)。阴道分娩组并发症发生率(7.50%)低于剖宫产组(27.50%),差异具有统计学意义(P<0.05)。阴道分娩组新生儿Apgar评分(8.67±1.46)分高于剖宫产组(7.24±1.26)分,差异具有统计学意义(P<0.05)。结论瘢痕子宫再次足月妊娠产妇选择经阴道分娩的分娩结局较好,建议产妇尽量选择阴道分娩。

Objective To explore the delivery method of uterine scar of full-term pregnancy again vaginal delivery. Methods A total of 80 pregnant women with uterine scar of full-term pregnancy again vaginal delivery were divided by different delivery methods into vaginal delivery group (selected vaginal delivery) and cesarean section group (selected cesarean section), with 40 cases in each group. Comparison were made on delivery bleeding volume, successful delivery rate, complications and neonatal Apgar score in two groups. Results The vaginal delivery group had less bleeding volume as (116.45±21.48) ml than (220.42±21.46) ml in the cesarean section group, and the difference had statistical significance (P<0.05). The vaginal delivery group had lower successful delivery rate (80.00%) than the cesarean section group (100.00%), and the difference had statistical significance (P<0.05). The vaginal delivery group had lower incidence of complications (7.50%) than the cesarean section group (27.50%), and the difference had statistical significance (P<0.05). The vaginal delivery group had higher neonatal Apgar score as (8.67±1.46) points than (7.24±1.26) points in the cesarean section group, and the difference had statistical significance (P<0.05). Conclusion Pregnant women with uterine scar of full-term pregnancy again vaginal delivery should select vaginal delivery, for its good delivery outcome, and they should be suggested for vaginal delivery.
