初发与复发溃疡性结肠炎院外治疗依从性的调查与分析

Investigation and Analysis of Extramural Hospital Treatment Compliance of Incipient and Relapse Ulcerative Colitis

目的:探讨溃疡性结肠炎初次发病和多次发病患者出院后饮食和药物治疗依从性的差异,了解溃疡性结肠炎复发率与治疗依从性的关系。方法:通过对181例溃疡性结肠炎初次发病与多次发病患者出院后1年的跟踪随访,以问卷调查表和门诊复诊所得资料,分析初次发病和多次发病患者饮食和药物治疗依从性的差异,并分析治疗依从性好坏与1年内累计复发率的关系。结果:饮食和药物治疗依从性好和总依从性好的比率初次发病患者明显少于多次发病患者,差异有统计学意义(P<0.05);1年内累计复发率初次发病患者明显高于多次发病患者(P<0.05)。结论:多次发病患者因反复多次发病,有长期痛苦体验,希望疾病早日消除的愿望极大,因而能严格执行医嘱,有良好的治疗依从性;初次发病患者因只发病1次,痛苦体验少,故出院后治疗依从性通常较差;同时溃疡性结肠炎的复发率与治疗依从性的好坏呈负相关。

Objective:To explore the differences of diet and medicine treatment compliance after hospitalization between incipient and relapse patients with UC.Methods:181patients with incipient and relapse UC after hospitalization were given follow-up for one year,data aquired by questionnaire and revisit,the differences of diet and medicine treatment compliance of incipient and relapse UC were analyzed,the relations between good or bad treatment compliance and accumulated recurrence rate within one year were analyzed.Results:The ratios of good diet and medicine treatment compliance and good overall compliance of incipient patients were less than these of the relapse patients(P<0.05);accumulated recurrence rate within one year of incipient patients were higher than that of the relapse patients(P<0.05).Conclusion:Because patients have many recurrences and long-term painful experience,they hope for eliminating diseases,they can follow the doctor's advice and they have good treatment compliance;the incipient UC patients only have once onset,less painful experience,so their treatment compliance worse after hospitalization;at the same time,recurrence rate of UC has negative correlation with good or bad treatment compliance.
