乳腺癌中膜型基质金属蛋白酶-1蛋白表达及其临床病理意义 被引量:2

摘要 目的探讨膜型基质金属蛋白酶-1(MT1-MMP)蛋白表达在乳腺癌发生、发展中的临床病理意义。方法收集2011年1月至2014年12月404例女性乳腺组织石蜡标本分为5组,其中浸润性乳腺癌组220例、导管原位癌(DCIS)Ⅲ组39例、DCISⅠ~Ⅱ组33例、普通型导管增生(UDH)组64例、正常乳腺组48例。采用免疫组化En Vision法检测MT1-MMP蛋白的表达。结果 (1)浸润性乳腺癌、DCISⅢ、DCISⅠ~Ⅱ、UDH、正常乳腺组中MT1-MMP蛋白阳性率分别为67.73%(149/220)、25.64%(10/39)、6.06%(2/33)、4.69%(3/64)、18.75%(9/48);浸润性乳腺癌组阳性率显著高于DCISⅢ、DCISⅠ~Ⅱ、UDH和正常乳腺组(均P<0.01);DCISⅢ组阳性率显著高于DCISⅠ~Ⅱ和UDH组(均P<0.05)。(2)MT1-MMP蛋白表达与腋窝淋巴结转移、TNM分期及孕激素受体(PR)相关(均P<0.05)。log is tic回归模型多因素分析显示浸润性乳腺癌患者MT1-MMP蛋白表达的独立相关因素包括腋窝淋巴结转移(OR=0.213,95%CI=0.084~0.544,P<0.01)和PR表达状态(OR=1.927,95%CI=1.049~3.542,P<0.05)。结论 MT1-MMP蛋白表达自UDH和DCISⅠ~Ⅱ、DCISⅢ至浸润性乳腺癌显著升高,且其MT1-MMP蛋白表达与浸润性乳腺癌患者多项预后不良因素相关,提示其在乳腺癌进展的全过程中均发挥了重要作用,有望成为抗乳腺癌治疗的一个潜在生物学标志。 Objective To investigate the expression of MT1-MMP protein in breast cancer and to evaluate its clinicopathologic significance. Methods The expression of MT1-MMP protein was detected by immunohistochemical EnVision method in 48 samples of normal breast tissues, 64 cases of usual ductal hyperplasia (UDH), 72 cases of ductal carcinoma in situ (DCIS) and 220 cases of invasive breast cancer. Results The expression rates of MT1-MMP protein in invasive breast cancer, DCIS I , DCIS l - I , UDH and normal breast tissues were 67.73/(149/220), 25.64/(10/39), 6.06/(2/33), 4.69/(3/64), 18.75/ (9/48). The MT1-MMP protein expression in invasive breast cancer was significantly higher than those in DCIS III , DCIS l - II , UDH and normal breast tissues (P<0.01), and MT1-MMP protein expression in DCIS III was significantly higher than those in DCIS l - II (P<0.05) and UDH (P<0.01). MT1-MMP protein expression in invasive breast cancer was increased with the increasing clinical stages (P<0.01), and MT1-MMP protein positive rates in patients with axillary lymph node metastasis and negative PR status were significantly higher than those with negative axillary lymph node and positive PR (P<0.01 and P<0.05, respectively). Logistic regression analysis showed that axillary lymph node metastasis (OR=0.213, 950/〇C7=0.084?0.544, P<0.01) and PR status (OR=1.927, 95°/〇C7=1.049?3.542, P<0.05) were independent factors of MT1-MMP protein expression in invasive breast cancer. Conclusion MT1-MMP protein expression shows significantly increasing trend from UDH, DCIS l - II , DCIS III to invasive breast cancer, and its expression is correlated with unfavorable prognostic factors of invasive breast cancer, suggesting that MT1-MMP may play an important role in the progression of breast cancer and might be a potential anti-cancer therapy biological marker.
