门诊西药房不合理用药处方的调查及结果分析

Investigation and analysis of irrational drug prescription in outpatient western pharmacy

目的调查分析门诊西药房不合理用药处方情况,以期指导临床合理用药。方法选取2014年1月~2016年1月我院门诊西药房处方25 000张,进行调查分析并探究门诊西药房处方不合理应用的原因。结果 25 000例门诊西药房处方中有450例为不合理用药处方,不合理应用率为1.8%。450例不合理用药处方的原因主要为剂量不合理、联合用药不规范、药物选择错误、给药方式不正确、间隔时间不合理等。剂量不合理处方有120例,联合用药不规范的处方有80例,选药不合理的处方有140例,给药方式不正确的处方有60例,给药间隔时间不合理的处方有50例。它们分别所占的比例为26.7%、17.8%、31.1%、13.3%、11.1%。其中80例联合用药不规范处方中,溶剂选择错误、重复用药、联合用药性质改变与药物联合应用效果差的处方分别有23例、24例、13例、20例,所占比例分别为5.1%、5.3%、2.9%、4.4%。140例选药不合理的处方中,药物剂型不对与特殊患者用药不当的分别有34例和46例,所占比例分别为7.6%、10.2%。结论为了患者用药安全、有效、经济及合理使用药品得到保障,进一步提高门诊西药房整体服务的水平,门诊药师必须要不断提升自身的审理处方的能力,建立好用药规范制度,对于不合理用药情况采取措施进行干预,提高门诊西药房合理用药率。

Objective To investigate the investigation and results of irrational prescription of western medicine pharmacy. Methods From January 2014 to January 2016, 25,000 cases of western medicine prescription were selected as the objects of study, and the reasons for the unreason able application of prescriptionof western medicine pharmacy were observed. Results Of the 25,000 outpatient western pharmacy prescriptions, 450 prescriptions were for irrational use,irrational application rate of 1.8%. 450 cases of irrational drug prescription for the main reason for the unreasonable dose, combined medication is not standardized,drug selection error, the wrong way of administration, interval time unreasonable and so on. There were 120 cases of irrational prescription, 80 cases of nonstandard prescription, 140 cases of irrational prescription, 60 cases of improper administration and 50 cases of irrational prescription. Accounting for 26.7%,17.8%, 31.1%, 13.3% and 11.1% respectively. There were 23 cases, 24 cases, 13 cases and 20 cases, respectively, in the combination of 80 cases of nonstandard prescription, solvent selection error, repeated medication, combined drug use and drug combination effect were 5.1% 5.3%, 2.9%, 4.4%. 140 cases of drug selection in the wrong prescription, the drug formulations are not appropriate for patients with special cases were 34 cases and 46 cases, respectively,the proportion was 7.6%, 10.2%. Conclusion In order to ensure the safety of patients, effective, economical and rational use of drugs to be protected, to further improve the overall level of outpatient western pharmacy overall service, pharmacists must continue to improve their ability to prescribe prescription, establish a good drug regulatory system for unreasonable drug use Measures to intervene to improve the rational rate of outpatient western pharmacy.
