摘要
目的检测心房颤动(atrial fibrillation,AF)与血清心型脂肪酸结合蛋白(heart type-fatty acid binding protein,HFABP)的临床相关性。方法实验组纳入105例AF患者,其中永久性房颤48例,阵发性房颤30例,持续性房颤27例。对照组为110例同期住院无房颤患者。所有纳入者均检测血浆H-FABP,同时测定心肌肌钙蛋白I(cardiac troponin I,cTnI)、高敏肌钙蛋白(hs-cTnI);肌酸激酶同工酶-MB(creatine kinase isoenzyme MB,CKMB),并做统计学分析。房颤患者出院后第1个月和3个月分别检验H-FABP浓度。结果 (1)H-FABP指标比较:AF组及亚组与对照组比较差异具有统计学意义(P<0.01,P<0.05);hs-cTnI指标比较:持续性房颤及永久房颤组与对照组比较差异具有统计学意义(P<0.05);cTnI,CKMB指标在各组间比较未见差异;(2)AF患者治疗后H-FABP水平下降,与治疗前比较差异均具有统计学意义(P<0.01,P<0.05),以阵发性AF组下降最明显。结论 H-FABP是检测AF患者存在心肌损伤较敏感指标,经过治疗后H-FABP水平下降,可能成为改善预后指标之一。
Objective To investigate relationship of heart type-fatty acid binding protein(H-FABP)in patients with atrial fibrillation.MethodsThe experimental group included105patients with AF.All were divided into three subgroups.The control group was110cases.H-FABP was measuredin all patients by ELISA one-step sandwich assay,at the same time,high sensitivity cardiac troponin I(hs-cTnI),cardiac troponin I(cTnI)andcreatine kinaseiso enzyme-MB(CK-MB)were determined.H-FABP were tested in1st month and3rd month in experimental group after the hospital.Results(1)There was significant difference in H-FABP between patients with atrial fibrillation and control group(P<0.01,P<0.05).The level ofhs-cTnI in paroxysmal AF group and persistent AF group were higher than in control group.While compared to the level of CK-MB and cTnI in twogroups,it had no statistical difference.(2)There was markedly difference on H-FABP level between before treatment and after treatment(P<0.01,P<0.05).H-FABP level declined obviously in paroxysmal AF group.Conclusion Serum H-FABP was a sensitive index to detect myocardial injuryin patients with atrial fibrillation.H-FABP may become one of the indicators for improving prognosis.
