2型糖尿病合并胆囊结石的相关危险因素分析

Analysis of risk factors associated with type 2 diabetes gallbladder stones

目的对2型糖尿病(T2DM)合并胆囊结石(GSD)的相关危险因素进行分析,以期为T2DM-GSD的预防提供参考。方法选取进行腹腔镜胆囊切除(LC)手术的GSD患者1 200例,其中合并T2DM的患者为168例,作为本次研究的实验组,另外抽取同期体检的单纯T2DM患者168例患者作为对照组。对两组患者的病例资料进行回顾性分析,包括SBP、DBP、空腹血糖(FBG)、TC、TG、HDL、LDL和体重指数(BMI)等因素,对上述因素进行单因素分析并对单因素分析有统计学差异的危险因素进一步进行多因素Logistic回归分析,从而得出T2DM-GSD的危险因素。结果两组患者年龄和性别差异无统计学意义。单因素分析显示,两组患者的SBP、FBG、TC、HDL和BMI等5个因素差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,SBP、TC、HDL和BMI是T2DM-GSD的独立危险因素。结论 T2DM-GSD的病理机制复杂,SBP、TC、HDL和BMI与T2DM-GSD的发生直接关联,这对往后临床上寻求T2DM-GSD的防治方法具有一定的借鉴意义。

Objective To analysis the risk factors associated with type2diabetes gallbladder stones(T2DM-GSD),in order to provide a reference for the prevention of the disease.Methods Take1200cases with GSD undergoing laparoscopic cholecystectomy(LC)surgery in our hospital from September2009to August2012,of which168patients combined with T2DM,as the experimental group in this study,taken168cases of patients examination in the same period in our hospital with T2DM alone as a control group.The clinical data of two groups were retrospectively analyzed,including SBP,DBP,fasting blood glucose(FBG),TC,TG,HDL,LDL,and body mass index(BMI),multivariate analysis of the above factors,further multivariate analysis the factors showed statistically significant difference in univariate analysis in order to identify the risk factors of T2DM-GSD.Results The differences of Age and gender of the two groups were not statistically significant(P<0.05).Univariate analysis showed that five factors involve SBP,FBG,TC,HDL,and BMI in the two groups have a statistically significant difference(P<0.05).Logistic regression analysis showed that,SBP,TC,HDL and BMI is the independent risk factors of T2DM-GSD.Conclusion The pathological mechanism of T2DMGSD is complexity,and SBP,TC,HDL,and BMI directly related to the occurrence of T2DM-GSD,which has a certain significance for later clinical sought of prevention method to T2DM-GSD.
