摘要
目的:探讨颧骨颧弓联合下颌角整形术后颞颌关节髁状突位置的变化。方法:选取2014年1月-2016年7月颧骨弓肥大合并下颌角肥大的50例就医者,均于笔者医院行颧骨颧弓联合下颌角整形术,观察手术对颞颌关节髁状突的影响。结果:50例就医者手术均顺利完成,术后切口均Ⅰ期愈合,并发症发生率为6.0%;术后6个月时,研究对象髁突内侧缘间距为(8.89±0.52)mm,髁突外侧缘间距为(12.74±0.48)mm,与术前的(8.93±0.43)mm、(12.71±0.52)mm比较,差异无统计学意义(P>0.05);50例就医者术后6个月Spee曲率半径有所减小,与术前数据比较,差异有统计学意义(P<0.05);50例就医者均对手术的美容效果满意,临床总满意度为100.00%。结论:颧骨颧弓联合下颌角整形术用于颧骨弓肥大合并下颌角肥大者,基本不影响髁状突位置,面部美容效果佳。
Objective To investigate the changes of condylar position of the temporomandibular joint after the operation of zygomatic arch combined with mandibular angle plastic surgery.Methods50cases of zygomatic arch hypertrophy and mandibular angle hypertrophy in January2014in our hospital were selected.All the patients were treated with zygomatic arch combined with mandibular angle plastic surgery in our hospital in the year of2016.Results50cases of surgery were successfully completed,the incisions were healed in stage I postoperative complication rate was6%;6months after operation,the object of study of condylar medial edge spacing(8.89±0.52)mm,condylar lateral edge spacing(12.74±0.48)mm,and before the operation.(8.93±0.43)mm,(12.71±0.52)mm comparison,the difference was not statistically significant(P>0.05);50cases of postoperative6months Spee radius decreases,and the data was compared with the preoperative,the difference was statistically significant(P<0.05);50cases were the cosmetic result of the operation to satisfied with the attitude of clinical total satisfaction was100%.Conclusion T he application of zygomatic arch combined with mandibular angle plastic surgery for the patients with zygomatic arch hypertrophy and mandibular angle hypertrophy has no influence on the condylar position,and the facial cosmetic effect is good.
