前方牵引矫治安氏Ⅲ类错的临床及美学效果

Clinical and Aesthetic Effect of Protraction Class Ⅲ Malocclusion

目的:探讨采用前方牵引方案治疗安氏Ⅲ类错的临床疗效及美学效果,为今后临床此类患者的矫正提供更科学的指导和参考。方法:选择2014年1月-2014年12月在笔者科室治疗的26例安氏Ⅲ类错牙合患者作为观察对象,对所有病例进行SNA、SNB、ANB、上中-NA、下中-NB、上下中切牙角、SN-MP、FH-MP、Y轴角的测量,对比矫正前后的数据。同时测量上气道项目,包括PNS-UPW、PNS-Ad2、OAW1、OAW2、OAW3、SPP-SPPW、U-MPW、V-LPW;对面部美容指标(ALs-FH、BLi-FH、ALs-BLi、Li-b-Pos、Ls-REP、Li-REP、Z角、Pg B-FH、Ns Pg-FH、G-Sn-Pg)进行观察。结果:在通过矫正前方牵引矫正治疗后,患者的SNA角、ANB角、SNB角、上中-NA角、下中-NB角、SN-MP角、FH-MP角要明显优于矫正前,差异具有显著性(P<0.05);鼻咽段PNS-UPW、PNS-Ad2治疗后变化明显,较治疗前差异显著,具有统计学意义(P<0.05);治疗后ALSFH、BLi-FH、ALs-BLi、Ls-REP、Li-REP、Z角、Pg B-FH和G-Sn-Pg相比与治疗前存在显著的统计学差异(P<0.05)。结论:前方牵引方案治疗安氏Ⅲ类错疗效确切,患者满意,保证了患者上颌骨、下颌骨正常的生长状态,操作简单,起效快,值得推广。

Objective To explore the clinical effects on the treatment of Angle ClassⅢmalocclusion for maxillary protraction program.To provide more scientific guidance and reference on orthodontic treatment for such patients in the future.Methods Choose between January2014and December2014,26patients with angle ClassⅢmalocclusio in our hospital as research object,Measure SNA,SNB,ANB,up middle-NA,own middle-NB,up middle down tooth angle,SN-MP,FH-MP,Y axis Angle of all cases,contrast the data before and after the correction.At the same time to measure airway project,including the PNS UPW,PNS Ad2,OAW1,OAW2,OAW3,SPP-SPPW,U-MPW,V-LPW.Results Through orthodontic traction ahead treatment,the degrees of SNA Angle,ANB Angle,SNB Angle,up middle-NA,down middle-NB,SN-MP Angle,FH-MP Angle were significantly better than before,the difference was significant(P<0.05);PN-UPW,PNS-Ad2ofnasopharyngeal period obviously changes after treatment,Compared with before treatment,the difference was statistically significant(P<0.05).Conclusion The curative effect of maxillary protraction program on the treatment of Angle ClassⅢmalocclusion is exact,satisfactory,that can ensue the maxillary,mandibular normal growth state,the operation is simple,work fast,and is worth promoting.
