环磷酰胺联合泼尼松在系统性红斑狼疮治疗中的疗效及安全性分析

Analysis on curative effect and safety of cyclophosphamide combined with prednisone in the treatment of systemic lupus erythematosus

目的探究环磷酰胺联合泼尼松在系统性红斑狼疮治疗中的疗效及安全性。方法 98例系统性红斑狼疮患者,按随机数表法分为对照组与研究组,每组49例。对照组患者给予泼尼松治疗,研究组患者给予环磷酰胺联合泼尼松进行治疗。检测并比较两组患者临床疗效、血清免疫球蛋白[免疫球蛋白(Ig)A、IgG、IgM]、补体C_3及C_4水平、不良反应发生情况。结果研究组患者治疗总有效率为93.9%,高于对照组的73.5%,差异具有统计学意义(P<0.05)。治疗前两组血清免疫球蛋白和补体C_3、C_4水平比较差异均无统计学意义(P>0.05);治疗1、3个月后,两组患者的血清免疫球蛋白水平均有所下降,补体C_3、C_4水平均有所升高,且研究组患者改善程度明显优于对照组,差异均具有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论环磷酰胺联合泼尼松是一种安全有效的治疗系统性红斑狼疮的方法 ,建议临床上推广应用。

Objective To explore the curative effect and safety of cyclophosphamide combined with prednisone in the treatment of systemic lupus erythematosus.Methods A total of98systemic lupus erythematosus patients were divided by random number table method into control group and research group,with49cases in each group.The control group received prednisone for treatment,and the research group received cyclophosphamide combined with prednisone for treatment.Detection and comparison were made on clinical curative effect,serum immunoglobulin[immunoglobulin(Ig)A,IgG,IgM],complement C3and C4level and occurrence of adverse reactions in two groups.Results The research group had higher total treatment effective rate as93.9%than73.5%in the control group,and the difference had statistical significance(P<0.05).Both groups had no statistically significant difference in serum immunoglobulin,complement C3and C4level before treatment(P>0.05).After1and3months of treatment,both groups had lower serum immunoglobulin level,and higher complement C3and C4level,and the research group had obviously better improvement than the control group.Their difference had statistical significance(P<0.05).Both groups had no statistically significant difference in incidence of adverse reactions(P>0.05).Conclusion Combination of cyclophosphamide and prednisone is a safe and effective method in treating systemic lupus erythematosus,and it is recommended to be promoted and applied in clinic.
