自锁托槽矫治器在牙颌畸形患者正畸中的应用价值

Application value of self-ligation appliance in orthodontics of dental-occlusal deformity patients

目的对牙颌畸形患者正畸治疗中自锁托槽矫治器应用价值进行探讨。方法 172例牙颌畸形患者,随机分为对照组和观察组,每组86例。观察组应用自锁托槽矫治器治疗,对照组应用传统金属托槽治疗,对两组患者的治疗效果进行比较。结果治疗7个月后,观察组患者牙龈指数、菌斑指数、龈沟出血指数均低于对照组,差异有统计学意义(P<0.05)。观察组矫治初期疼痛视觉模拟量表(VAS)平均分为(1.2±0.2)分,低于对照组的(6.8±0.4)分,差异有统计学意义(P<0.05)。观察组治疗1、3、5年后牙齿脱落率分别为0、2.3%(2/86)、11.6%(10/86),对照组分别为9.3%(8/86)、14.0%(12/86)、23.3%(20/86);观察组牙齿脱落率低于对照组,差异有统计学意义(P<0.05)。放置矫治器24、72 h后,观察组患者龈沟液天门冬氨酸转氨酶(AST)含量低于对照组,差异有统计学意义(P<0.05)。结论牙颌畸形患者正畸中应用自锁托槽矫治器治疗,能够明显改善牙周指数,降低矫正初期疼痛程度和托槽脱落率,值得进行推广。

Objective To explore the application value of self-ligation appliance in orthodontics of dental-occlusal deformity patients.Methods A total of172dental-occlusal deformity patients were randomly divided into control group and observation group,with86cases in each group.The observation group received self-ligation appliance for treatment,and the control group received traditional metal brackets for treatment.Comparison were made on therapeutic effect in two groups.Results After7months of treatment,the observation group had lower gingival index,plaque index,gingival sulcus bleeding index than the control group,and the difference had statistical significance(P<0.05).The observation group had lower mean visual analogue scale as(1.2±0.2)points at initial stage of correction than(6.8±0.4)points in the control group,and the difference had statistical significance(P<0.05).The observation group had tooth loss rate respectively as0,2.3%(2/86)and11.6%(10/86)after1,3and5years of treatment,which were respectively as9.3%(8/86),14.0%(12/86)and23.3%(20/86)in the control group.The observation group had lower tooth loss rate than the control group,and the difference had statistical significance(P<0.05).After24and72h of appliance placement,the observation group had lower gingival crevicular fluid aspartate aminotransferase(AST)content than the control group,and the difference had statistical significance(P<0.05).Conclusion Application of self-ligation appliance in orthodontics of dental-occlusal deformity patients can obviously improve periodontal index,reduce pain degree at initial stage of correction and expulsion rate of bracket,and it is worth promotion.
