探究换血对新生儿高胆红素血症内环境血液学指标的影响

Exploration of impact of exchange transfusion on internal environment haematological indexes of neonatal hyperbilirubinemia

目的观察与分析换血治疗对新生儿高胆红素血症内环境血液学指标的影响。方法 42例新生儿高胆红素血症患儿作为研究对象,对全部患儿予以换血治疗,并对其治疗前后的内环境血液学指标变化情况加以观察与对比。结果换血后患儿的红细胞、血红蛋白、红细胞比容与换血前比较,差异均无统计学意义(t=-0.518、-1.884、-1.520,P>0.05);换血后的白细胞、血小板均低于换血前,差异均具有统计学意义(t=7.539、11.547,P<0.05)。换血后患儿的总胆红素、直接胆红素、间接胆红素、血糖水平与换血前比较,差异均有统计学意义(t=11.509、3.788、11.000、-6.898,P<0.05)。换血后患儿的钠、钾、氯水平均优于换血前,差异均有统计学意义(t=17.426、12.926、6.255,P<0.05);换血前后钙水平比较,差异无统计学意义(t=0.275,P>0.05)。结论对新生儿高胆红素血症患儿采取换血治疗,效果显著,但在治疗过程中,需密切监测患儿的各项生命体征或血液学指标,从而确保疾病治疗的效果。

Objective To observe and analyze the impact of exchange transfusion on internalenvironment haematological indexes of neonatal hyperbilirubinemia.Methods A total of42newborns withhyperbilirubinemia as study subjects all received exchange transfusion treatment,and their internal environmenthaematological indexes changes before and after treatment was observed and compared.Results After exchangetransfusion,newborns had no statistically significant difference in red blood cells,hemoglobin and hematocritvalue comparing with those before exchange transfusion(t=-0.518,-1.884,-1.520,P>0.05).After exchangetransfusion,newborns had lower white blood cells and platelets than those before exchange transfusion,andtheir difference had statistical significance(t=7.539,11.547,P<0.05).After exchange transfusion,newborns hadstatistical significance in total bilirubin,direct bilirubin,indirect bilirubin,blood glucose levels,comparing with before exchange transfusion(t=11.509,3.788,11.000,-6.898,P<0.05).After exchange transfusion,newbornshad lower sodium,potassium and chlorine levels than those before exchange transfusion,and their differencehad statistical significance(t=17.426,12.926,6.255,P<0.05).There was no statistically significant differencein calcium level before and after exchange transfusion(t=0.275,P>0.05).Conclusion Exchange transfusionshows significant effect in hyperbilirubinemia newborns,but vital signs or hematology indicators should be closelymonitored in the process of treatment,so as to ensure the therapeutic effect of disease.
