曲安奈德对白内障术后前葡萄膜炎患者的疗效及炎症因子的影响

Effect of Triamcinolone Acetonide on Curative Effect and Inflammatory Factors in Patients with Anterior Uveitis after Cataract Surgery

目的:观察曲安奈德对白内障术后前葡萄膜炎患者的疗效及炎症因子的影响。方法:选取2015年4月至2016年4月郑州大学附属洛阳中心医院眼科收治的白内障术后前葡萄膜炎患者78例,采用随机数表法分组,各39例。对照组予以平衡盐溶液治疗,观察组在对照组基础上予以曲安奈德治疗。观察对比两组视力恢复效果及炎症因子[白介素-6(Interleukin-6,IL-6)、白介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]水平变化情况。结果:手术后观察组视力恢复明显优于对照组,差异有统计学意义(P<0.05);手术前两组眼压对比,差异无统计学意义(P>0.05),手术后1周、手术后1个月两组眼压均降低,观察组低于对照组,差异有统计学意义(P<0.05);手术前两组炎症因子水平对比,差异无统计学意义(P>0.05),手术后观察组IL-6、IL-1β及TNF-α水平均低于对照组,差异有统计学意义(P<0.05);手术后1d、手术后1周观察组前房炎症反应"+"度占比51.28%(20/39)、97.44%(38/39)高于对照组25.38%(9/39)、56.41%(27/39),差异有统计学意义(P<0.05)。结论:曲安奈德治疗白内障术后前葡萄膜炎患者疗效显著,能有效恢复患者视力,降低眼压,抑制炎症因子表达,减轻炎症反应。

Objective:To observe the effect of triamcinolone acetonide on the efficacy and inflammatory factors of patients with anterioruveitis after cataract surgery.Methods:From April2015to April2016,78patients with anterior uveitis after cataract surgery in thehospital were randomly divided into control group and observation group(n=39).The control group was treated with balanced saltsolution,and the observation group was treated with triamcinolone acetonide on the basis of the control group.Vision recovery effectand inflammatory factor interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)levels in the two groups wereobserved and compared.Results:After treatment,the visual recovery of the observation group was significantly better than that of thecontrol group(P<0.05);There was no significant difference in intraocular pressure between the two groups before operation(P>0.05),1week and1month after the surgery,the intraocular pressure of both groups decreased,the observation group was lower than the controlgroup,the difference was statistically significant(P<0.05);There was no significant difference in inflammatory factors levels betweenthe two groups before treatment(P>0.05),but after treatment,the levels of IL-6,IL-1βand TNF-αin the observation group were lowerthan those in the control group,the difference was statistically significant(P<0.05).At1day and1week after operation,the anteriorchamber inflammation response“+”rate was51.28%(20/39)and97.44%(38/39)in the observation group,which was higher thanthat in the control group(9/39),56.41%(27/39),the difference was statistically significant(P<0.05).Conclusion:Treatment of anterioruveitis after cataract surgery using triamcinolone acetonide gets a significant clinical effect,and it can effectively restore the patient'svision,reduce intraocular pressure,inhibit the expression of inflammatory factors and reduce inflammatory response.
