家庭医生签约对社区高血压患者血压控制的效果分析

Analysis of the effect of family doctor signing on blood pressure control of hypertension patients in community

目的:探讨分析家庭医生签约对社区高血压患者血压控制的效果。方法:收治高血压患者120例,并进行1年的随访管理,对管理前后的血压及相关因素进行分析。结果:实施家庭医生签约后,患者对高血压相关知识的知晓率、治疗的依从性及血压控制较实施前有明显改善,差异有统计学意义(P<0.05)。结论:家庭医生签约可将慢病管理窗口前移,显著改善患者的血压水平。

Objective:To explore the effect of family doctor signing on blood pressure control of community hypertension patients.Methods:120hypertension patients were selected.They were followed up for1year.The blood pressure and related factors wereanalyzed before and after management.Results:After the implementation of family doctor signing,the patients'awareness ofhypertension related knowledge,treatment compliance and blood pressure control were significantly improved,and the differenceswere statistically significant(P<0.05).Conclusion:Family doctor signing can move forward the management window of chronicdisease,significantly improve the level of blood pressure in patients.
