摘要
目的:观察不同胰岛素强化治疗对初诊2型糖尿病患者血清氧化应激水平及胰岛素抵抗的影响,为临床治疗2型糖尿病提供参考。方法:选取河南科技大学第三附属医院2013年8月至2016年6月收治的74例初诊2型糖尿病患者,根据治疗方法不同分为观察组(n=43)和对照组(n=31),观察组给予胰岛素泵治疗,对照组给予多次皮下注射胰岛素治疗,比较两组SOD、MDA、RGSH、空腹血糖、餐后2h血糖、GA、FIN、HOMA-IR、HOMA-β指标水平及血糖达标时间。结果:治疗后观察组SOD高于对照组,MDA低于对照组,差异具有统计学意义(P<0.05);治疗后观察组空腹血糖、餐后2h血糖、GA低于对照组,差异具有统计学意义(P<0.05);治疗后观察组FIN高于对照组,差异具有统计学意义(P<0.05);治疗后观察组HOMA-IR低于对照组,HOMA-β高于对照组,差异具有统计学意义(P<0.05);观察组血糖达标时间低于对照组,差异具有统计学意义(t=2.955,P<0.05)。结论:胰岛素泵强化治疗可改善初诊2型糖尿病患者血清氧化应激水平和胰岛素抵抗状态,有效控制血糖水平,提高胰岛β细胞功能,具有较高的实用价值,值得推广和应用。
Objective:To observe the effect of different insulin intensive therapy on serum oxidative stress and insulin resistance in newly diagnosed type2diabetes mellitus,and to provide reference for clinical treatment of type2diabetes mellitus.Methods:Selected74newly diagnosed type2diabetic patients in Third Affiliated Hospital Oriental Hospital of Henan University of Science and Technology from August2013to June2016were divided into observation group(n=43)and control group(n=31)according to different treatment methods.The observation group was treated with insulin pump,the control group was given multiple subcutaneous insulin therapy,SOD,MDA,RGSH,fasting blood glucose,2h postprandial blood glucose,GA,FIN,HOMA-IR,HOMA-βindicator levels and blood glucose standard time were compared between the two groups.Results:After treatment,SOD in the observation group was higher than in the control group,MDA was lower than the control group,(P<0.05);After treatment,fasting blood glucose,postprandial2h blood glucose,GA in the observation group were lower than the control group(P<0.05);After treatment,the FIN of the observation group was higher than that of the control group(P<0.05);HOMA-IR in the observation group was lower than the control group,HOMA-βwas higher than the control group after treatment(P<0.05);The blood glucose standard time in the observation group was significantly lower than the control group(P<0.05).Conclusion:Insulin pump intensive therapy can improve the serum oxidative stress level and insulin resistance in patients with newly diagnosed type2diabetes mellitus,and effectively control blood glucose level and improve pancreaticβcell function,thus it is of great practical value and worthy of popularization and application.
