泉州市白内障复明手术人工晶体度数配置状况分析 被引量:2

摘要 目的调查白内障复明手术患者的人工晶体(IOL)置入情况,从而优化人工晶体配置。方法统计2012~2013年在福建省泉州市进行的2 692例白内障复明手术患者,记录人工晶体置入前度数测量值、术中置入度数和患者置入后1 d的5 m远视力,计算置入人工晶体与测量值的差值,置入后1 d的视力与术前视力的差值。分析人工晶体度数的分布状态,植入人工晶体度数与测量值之间的差异,及其对术后术前视力差值的影响。结果白内障复明手术人工晶体测量值的分布状态:最小值-9 D,最大值34.5 D。<10 D频率为0.6%;10 D≤IOL<15 D频率为2.4%;15 D≤IOL<18 D频率为3.2%;18 D≤IOL<20 D频率为15.1%;20 D≤人工晶体度数<22 D频率为41.3%;22 D≤IOL<24 D频率为30.6%;24 D≤IOL<26 D频率为6.2%;26 D≤IOL≤34.5 D频率为0.6%。其中以21.0 D,20.0 D和22.0 D最多,频率分别为12.5%,11.6%,和10.7%。术中置入人工晶体度数与置入前测量值的差值完全符合(差值为0)者2 340例,占87%,术后视力平均提高4.342行;0.5 D≤相差值≤2.0 D者300例,占11%,术后视力平均提高3.125行,>2.0 D者52例,占2%,术后视力平均提高2.010行。结论人工晶体植入度数的准确性影响白内障术后的恢复视力。在泉州市白内障复明手术中人工晶体总量及常用度数晶体量的配置基本合理,但大部分≤10 D及≥27 D的人工晶体缺如,应根据人工晶体测量度数的频数分布进一步改善人工晶体的配置。 Objective To optimize the distribution of intraocular lens(IOL)diopter by investing IOL in cataract surgery for prevention of blindness.Methods A total of2692patients from Quanzhou City,Fujian province,from2012to2013were studied.Their IOL diopter values measured before operation,diopter values of IOL practically transplanted,and visual acuity on the first day after operation were recorded.The diopter value distribution of patients'IOL,the difference between measured and transplanted lens diopter,and its effect on the patients'visual acuity after operation were analyzed.Results The lowest IOL diopter value measured before operation was-9D,the largest one was34.5D.The frequency of different diopter values:<10D:0.6%;10D≤IOL<15D:2.4%;15D≤IOL<18D:3.2%;18D≤IOL<20D:15.1%;20D≤IOL<22D:41.3%;22D≤IOL<24D:30.6%;24D≤IOL<26D:6.2%;26D≤IOL≤34.5D:0.6%.And the21.0D,20.0D and22D up,frequencies were12.5%,11.6%,and10.7%.Among all the cases,There were2340cases,accounting for87%with no difference found between measured diopter and that of practically transplanted,the postoperative visual acuity increased an average of4.342lines;300cases,11%with difference between0.5D and2D,the visual acuity increased an average of3.125lines and52cases,2%above2D,the visual acuity increased an average of2.010lines.Conclusion Implantation of IOL degree affects the accuracy of the recovery of visual acuity after cataract surgery.The numbers of total IOL series and frequently used lOLs prepared before operation are reasonable in Quzhou City.But most lOLs with diopters of≤10D and≥27D were absence.Therefore,it is necessary to improve the distribution of intraocular lens diopter in cataract surgery for prevention of blindness.
