肝内周围型胆管细胞癌的CT和MRI诊断及病理基础探究

CT and MRI diagnosis and pathological basis of intrahepatic peripheral cholangiocarcinoma

目的探讨研究对肝内周围型胆管细胞癌患者进行CT和MRI诊断方法,分析其病理基础。方法随机选取本院收治的肝内周围型胆管细胞癌患者42例作为研究对象。所有患者均进行CT扫描和MRI检查诊断,分析患者影像学特征,并对单纯CT诊断、单纯MRI诊断及CT联合MRI诊断的符合率进行比较分析,探讨最为有效的诊断措施。结果 42例肝内周围型胆管细胞癌患者CT平扫显示肿块为低密度,且其中36例患者病灶边界不清晰;MRI扫描发现肿块在T2WI表现为不均匀性高信号,而在T1WI则表现为均匀性信号;其中37例患者增强扫描发现病灶边缘强化;患者病灶平均直径为(5.39±0.25)cm,漏诊2例。CT诊断方法符合率为76.19%,MRI诊断方法符合率为78.57%,CT联合MRI诊断方法符合率为95.24%,单纯CT诊断和单纯MRI诊断符合率均显著低于CT联合MRI诊断符合率,数据比较差异具有统计学意义(P<0.05)。结论在肝内周围型胆管细胞癌患者诊断中应用CT联合MRI诊断措施,能够进一步提高早期病症诊断率,确保患者实现尽早治疗,提高治疗效果,即CT联合MRI值得在临床上推广应用。

Objective To investigate the CT and MRI diagnosis of intrahepatic peripheral cholangiocarcinoma patients studied,analysis of the pathological basis.Methods Patients were randomly selected in our hospital from intrahepatic peripheral cholangiocarcinoma as the research object,a total of42patients.All patients were CT scan and MRI examination,characteristics of patients with image analysis,and the simple CT diagnosis,simple MRI diagnosis and CT combined with MRI diagnostic coincidence rates were compared,to explore the most effective measures for diagnosis.Results42cases of intrahepatic peripheral cholangiocarcinoma Patients with CT scan showed low density mass,and36cases of boundary is not clear;MRI scan showed the tumor in T2WI showed inhomogeneous high signal in T1WI and showed the uniformity of signal;37patients scan showed the lesion edge enhancement;lesions in patients with an average diameter of(5.39±0.25)cm,2cases of misdiagnosis of.CT diagnosis coincidence rate was76.19%,MRI diagnosis coincidence rate was78.57%,CT combined with MRI diagnosis coincidence rate was95.24%,the rate was significantly lower than that of C with pure CT and pure MRI diagnosis diagnosis The coincidence rate of diagnosis of MRI combined with T,statistically significant data difference(P<0.05).Conclusion The application of CT combined with MRI diagnostic measures in patients with diagnosis of intrahepatic peripheral cholangiocarcinoma,can further improve the rate of early diagnosis,to ensure that patients achieve early treatment,improve the therapeutic effect of CT combined with MRI,which is worthy of popularization and application in the clinic.
