摘要
目的探究瑞芬太尼复合丙泊酚对支撑喉镜下声带息肉摘除术患者血流动力学的影响。方法随机抽取行支撑喉镜下声带息肉摘除术的患者80例,观察组予瑞芬太尼复合丙泊酚静脉麻醉,对照组予芬太尼复合丙泊酚静脉麻醉,比较两组患者麻醉诱导前、插入支撑喉镜后5 min及拔出喉镜后1 min的血流动力学指标,丙泊酚总用量、意识消失时间、麻醉苏醒时间和麻醉不良反应发生情况。结果经对比发现,插入支撑喉镜后5 min及拔出喉镜后1 min对照组患者SBP、DBP较麻醉诱导前明显升高,HR增快(P<0.05),插入支撑喉镜后5 min及拔出喉镜后1 min对照组SBP、DBP较观察组显著升高,HR明显增快(P<0.05)。观察组患者术中丙泊酚总用量(102.4±30.8)mg少于对照组(165.6±31.5)mg,观察组患者麻醉后意识消失时间(10.9±4.2)min及停药后苏醒时间(4.5±1.6)min均短于对照组(18.6±4.7)min和(7.1±2.4)min。对照组患者麻醉不良反应总发生率(15.0%)明显高于观察组(2.5%),差异均有统计学意义(P<0.05)。结论瑞芬太尼复合丙泊酚静脉麻醉应用于支撑喉镜下声带息肉摘除手术具有良好的临床效果。
Objective To explore the effect of remifentanil combined with propofol on hemodynamics in patients with vocal cord polyps under suspension.Methods Randomly selected from laryngoscope vocal cord polyp in80cases of patients,the observation group were given remifentanil combined with propofol intravenous anesthesia,the control group was treated with fentanyl and propofol intravenous anesthesia,compared two groups of patients with anesthesia induced before and after insertion of5min laryngoscope and pull out the hemodynamic index of1min the laryngoscope,the total dosage of propofol,consciousness loss time,anesthesia time and anesthesia adverse reactions.Results By comparison,insert and pull out the5min laryngoscope after laryngoscopy after1min patients in the control group SBP,DBP compared with before induction of anesthesia was significantly increased,HR increased(P<0.05),5min laryngoscope after insert and pull out the laryngoscope after1min control group SBP and DBP were significantly higher than those in the observation group,HR was significantly faster(P<0.05).The total amount of propofol(102.4±30.8)mg in the observation group was less than that in the control group(165.6±31.5)mg,the observation group was less than the control group after the consciousness disappeared time(10.9±4.2)min and the recovery time(4.5±1.6)min was shorter than the control group(18.6±4.7)min and(7.1±2.4)min.The incidence of adverse reactions in the control group(15.0%)was significantly higher than that in the observation group(2.5%).Conclusion Remifentanil Combined with propofol intravenous anesthesia in the treatment of vocal cord polyps with a good clinical effect.
