枸橼酸咖啡因与氨茶碱治疗早产儿原发性呼吸暂停的疗效与安全性 被引量:3

摘要 目的研究原发性呼吸暂停的早产儿行氨茶碱和枸橼酸咖啡因治疗的临床疗效及安全性,以达到缓解患儿病情和改善预后的目的。方法选取原发性呼吸暂停60例早产儿予以回顾性地分析,按照临床所用不同治疗方案分成两组,将行单纯氨茶碱治疗30例患儿作为对照组,将行单纯枸橼酸咖啡因治疗30例患儿作为观察组,采用SPSS22.0统计软件对两组临床疗效及安全性进行对比。结果两组Apgar评分比较差异无统计学意义;观察组血浆β-EP水平(136.02±23.50)ng/L比对照组(220.48±36.80)ng/L优,且平均用氧时间(4.20±2.06)d、呼吸暂停终止时间(2.20±1.02)d及呼吸暂停持续时间(25.50±5.20)s、发作次数(2.50±0.60)次、症状消除时间(43.02±4.40)h均比对照组少(P<0.05);观察组在两组治疗后HR、SpO_2、PaO_2均改善基础上,其改善幅度均比对照组显著(P<0.05)。结论原发性呼吸暂停的早产儿行枸橼酸咖啡因治疗能够缓解病情,改善患儿呼吸暂停状况,优化患儿血浆β-EP水平及HR、SpO_2等指标,安全性高,预后效果佳,具有临床推广和应用价值。 Objective To study the primary apnea of premature infants line of aminophylline and caffeine citrate treatment clinical curative effectand security,to achieve the purpose of ease cases and improve the prognosis.Methods select60cases of premature infants with primary apnearetrospectively analysis,used different according to the clinical treatment plan is divided into two groups,the row pure aminophylline treatment of30cases as control group,the row pure caffeine citrate in the treatment of30cases as the observation group,using SPSS22.0statistical software werecompared on two groups of clinical curative effect and security.Results The Apgar score is similar between the two groups have no statistical significance;Plasma beta EP level observation group(136.02±23.50)ng/L than the control group(220.48±36.80)ng/L,and the average time(4.20±2.06)d with oxygen,apnea end time(2.20±1.02)d and apnea duration(25.50±5.20)s,the number of attacks(2.50±0.60),symptoms to eliminatetime(43.02±4.40)h was less than control group(P<0.05);Observation group in the two groups after treatment based on HR,SpO2,PaO2were improved,the improvement than the control group significantly(P<0.05).Conclusion Primary apnea of premature infants caffeine citrate therapycan alleviate,improve children apnea,optimization with plasma beta EP level and HR,SpO2and other indicators,high safety,good prognosis effect,clinical popularization and application value.
