摘要 目的:为探讨迟发型痤疮的发病机制及影响因素,将门诊诊治的40例迟发性痤疮临床总结。方法:将门诊收治的40例迟发性痤疮按Pillsbury法评估分为四类,受试者均在月经期的第2天采血,于早晨8︰00~9︰00空腹抽血3 m L,分离血清置-21℃冰箱中保存待检。所检测的性激素分别为睾酮、雌二醇、孕酮、促卵泡素、黄体生成素。具体操作:待检血清上机(全自动电化学发光免疫分析仪)后,仪器将自动进行检测分析,并打印出结果。采用t检验进行统计学处理。目的:通过实验室结果,分析各种因素对迟发型痤疮的影响,从而得出其发病机制及影响因素。结论:女性迟发性痤疮患者是可能与现代女性生活节奏快、长期精神过度紧张、睡眠欠佳刺激垂体—肾上腺轴,引起卵巢功能不够完善,雌二醇分泌较少,睾酮/雌二醇比值升高,雄激素水平相对升高致使痤疮发生或加重。我们推测可能是由于患者血中雌激素水平下降,对腺垂体的反馈抑制作用减弱所致这与以往的某些报道认为是雄性激素升高引起迟发性痤疮是不一致的。 Objective:In order to explore the pathogenesis and effect of tardive acne,40cases of clinical diagnosis and treatment of tardive acne were clinically summarized.Methods:40patients with tardive acne were divided into four groups according to the Pillsbury method.The subjects received blood samples from the second days of menstruation,and the fasting blood was3mL at8:00am to9:00am.The serum was separated and stored at21DEG C in the refrigerator for examination.The tested hormones included testosterone,estradiol,progesterone,FSH,LH.Specific operation:after the examination of serum on the machine(fully automated electrochemical immunoassay analyzer),the instrument would automatically detect,analyze and print the results.T test was used for statistical treatment.The objective was to clarify various factors on tardive acne by analyzing the laboratory results,the pathogenesis and influencing factors.Conclusion:Women with acne are closely related to the modern woman fast-paced life,mental strain,poor sleeping for a long time to stimulate the pituitary-adrenal axis,which caused the imperfect ovarian function,less secretion of estradiol,increased testosterone/estradiol ratio,which finally led to the relatively elevated Androgen levels or worsened the acne.We speculated that it may be due to the decreased estrogen levels in the blood of patients and the feedback on the anterior pituitary gland due to inhibitory effect of this was lessened,our speculation was inconsistent with previous report,which held that tardive acne was induced by androgen.
