密集型银质针联合肋间神经射频热凝术治疗带状疱疹后神经痛的疗效观察

Observation of therapeutic effect of intensive silver needle combined with intercostal nerve radiofrequency thermocoagulation on postherpetic neuralgia

目的:观察密集型银质针联合肋间神经射频热凝治疗带状疱疹后神经痛的疗效及其安全性。方法:30例带状疱疹后神经痛患者(VAS≥4.0),随机分为A、B两组,每组15例。A组行皮损区密集型银质针治疗;B组在密集型银质针治疗的基础上,联合肋间神经射频热凝进行治疗。采用视觉模拟评分法(VAS法),分别测评入院时、出院时及出院后6个月的VAS评分,并根据VAS改善程度统计疗效(分为显效、有效和无效)。结果:出院时及出院后6个月时,A、B两组的VAS评分均较入院时有显著降低(P<0.01);两组间VAS评分比较,出院时B组较A组显著性降低(P<0.05)。出院后6个月时B组较A组显著性降低(P<0.05)。治疗后6个月总有效率单纯银质针组为53.7%,银质针联合射频组为98.6%;显效率银质针联合射频组为38.8%,单纯银质针组为76.3%。银质针联合射频组的总有效率和显效率均显著高于单纯银质针治疗组(P<0.01)。结论:密集型银质针联合肋间神经射频热凝术能有效缓解带状疱疹后神经痛,疗效优于单纯银质针治疗。

Objective:To observe the therapeutic effect of intensive silver needle combined with intercostal nerve radiofrequency thermocoagulation on postherpetic neuralgia.Methods:30cases of elderly hypertensionpostherpetic neuralgia patients(VAS≥4.0)were randomly divided into two groups.Group A received dense silver needle treatment;Group B received intensive silver needle combined with intercostal nerve radiofrequency thermocoagulation on the basis of the treatment of Group A.Visual analogue scale(VAS)was used to assess the VAS scores at admission,at discharge and6months after discharge,and to evaluate the efficacy according to the VAS improvement(divided into marked,effective and ineffective).Results:At discharge and6months after discharge,the VAS scores in two groups were significantly lower than those at admission(P<0.01);The VAS score between the two groups was compared,Group B was significantly lower that of group A(P<0.05).6months after discharge,Group B was significantly lower that of group A(P<0.05).After6months treatment,the total effective rate of Group A was53.7%and that of Group B was98.6%;The effective rate of two groups was38.8%and76.3%respectively,the total effective rate and the effective rate of Group B were significantly higher than those of Group A.Conclusions:Intensive silver needle combined with intercostal nerve radiofrequency thermocoagulation could effectively relieve the postherpetic neuralgia,its effect was better than the pure application of silver needle.
