半夏白术天麻汤联合桂哌齐特治疗后循环缺血性眩晕疗效观察

Observation on treating posterior circulation ischemic vertigo with the Banxia Baizhu Tianma decoction plus cinepazide

目的:观察半夏白术天麻汤联合桂哌齐特治疗后循环缺血性眩晕的临床疗效。方法:选择我院2015年1-12月收治的、符合入组标准的后循环缺血性眩晕患者70例作为研究对象,采用SPSS 19.0软件生成随机数字表,按就诊顺序分为观察组35例和对照组35例,对照组单纯静脉点滴桂哌齐特320mg,1次/d,14d为一个疗程,观察组在对照组的基础上口服半夏白术天麻汤,1剂/d,早晚分服,连续服用14d,治疗前后进行症状评分,采用TCD检测椎基底动脉血流速度,包括基底动脉平均血流速度(Vm)、收缩峰期血流速度(Vs)、舒张末期血流速度(Vd),比较两组临床疗效。结果:观察组和对照组治疗后头晕目眩、恶心呕吐、耳鸣或听力减退、倦怠乏力、汗出异常评分均较治疗前明显下降(P均<0.05),观察组治疗后耳鸣或听力减退、倦怠乏力、汗出异常评分明显低于对照组(P均<0.05);观察组和对照组治疗后Vm、Vs、Vd均较治疗前明显升高(P均<0.05),观察组治疗后Vm、Vs、Vd明显高于对照组(P均<0.05);观察组的临床总有效率明显高于对照组(P均<0.01)。结论:半夏白术天麻汤联合桂哌齐特治疗后循环缺血性眩晕疗效确切,能显著改善椎基底动脉血流速度,缓解临床症状,值得临床推广使用。

Objective:To observe clinical efficacy of the Banxia Baizhu Tianma decoction plus cinepazide on posterior circulationischemic vertigo.Methods:70cases were divided into the observation group and control group,35cases in each group.The control grouptook cinepazide.The observation group took the Banxia Baizhu Tianma decoction more.Results:After treatment,the scores of dizziness,nausea and vomiting,tinnitus or hearing loss,fatigue and sweating were significantly lower than before treatment(P<0.05).And the scoresof tinnitus or hearing loss,fatigue and sweating abnormality were significantly lower in the observation group(P<0.05).After treatment,levels of Vm,Vs and Vd were significantly higher than before treatment;and those in the observation group were better(P<0.05).Thetotal efficiency in the observation group was significantly higher than the control group(P<0.01).Conclusion:The Banxia Baizhu Tianmadecoction with cinepazide on ischemic vertigo is effective,by significantly improving the vertebrobasilar artery velocity and relievingclinical symptoms,and is worthy of promotion.
