宫腔镜对流产术后异常子宫出血的诊治分析

Diagnosis and treatment of abnormal uterine bleeding after abortion by hysteroscopy

目的探讨宫腔镜对流产术后异常子宫出血的诊疗效果。方法选取我院在2014年1月至2016年1月收治的590例因流产术后出现子宫出血的患者，对其采用宫腔镜进行诊疗的效果进行回顾性分析。结果宫腔镜检查结果显示，不全流产患者为550例，占总人数的93.2%，宫腔内无妊娠残留坏死组织的患者为40例，占总人数的6.78%。590例患者术中有589里患者出血3-10mL，占总数比例为99.8%，仅有1例出血量为100-120mL，占总数比例为0.02%。590例患者术后3天B超复查时有589例患者宫腔内无异常回声，1例患者宫腔内有异常回声；15天电话回访时所有患者均无异常阴道流血；35天电话回访时所有患者均有月经来潮现象。结论对于流产术后异常子宫出血患者，宫腔镜检查能够直接观察到宫腔内有无妊娠残留坏死组织，以及坏死组织残留的位置、大小，并根据宫腔镜的检查结果，有针对性的对病灶部位进行处理，从而达到微创、治疗快、恢复快的效果。

Objective To investigate the diagnosis and treatment effect of hysteroscopy on abnormal uterine bleeding after abortion.Methods From January2014to January2016,590patients with uterine bleeding after abortion were selected,and the effect of hysteroscopy on the diagnosis and treatment was retrospectively analyzed.Results Hysteroscopy showed550cases of incomplete abortion,accounting for93.2%of the total number.There were no residual intrauterine tissue in pregnancy and40cases in the uterine cavity,accounting for6.78%of the total number.In590patients,there were589miles of bleeding in3-10patients,accounting for99.8%of the total,only1cases,the amount of bleeding was100-120mL,accounting for0.02%of the total number of mL.590patients3days after ultrasound showed589cases of patients with intrauterine echo abnormal echo in1cases patients with uterine cavity;15day telephone interviews all the patients had no abnormal vaginal bleeding;35days of telephone visit when all patients had menstruation phenomenon.Conclusion For patients with abnormal uterine bleeding after abortion,hysteroscopy can directly observe the intrauterine pregnancy residues of necrotic tissue and necrotic tissue residue position and size,and according to the hysteroscopy examination results,for the treatment of the lesion,so as to achieve the treatment of minimally invasive,fast,fast recovery effect.
