中西医结合治疗重症胰腺炎45例

Treating 45 Cases of Severe Pancreatitis by Integrative Medicine

目的:观察在西医常规治疗的基础上采用大承气汤治疗重症胰腺炎的临床疗效。方法:将90例患者随机均分为观察组和对照组各45例。2组均给予常规治疗,对照组在常规治疗基础上应用呼吸机展开呼吸支持治疗,观察组在对照组治疗的基础上给予大承气汤治疗。观察2组临床疗效及腹痛消失时间、血清淀粉酶恢复时间、胃肠功能恢复时间及ICU住院时间,并评测急性生理学与慢性健康状况评分Ⅱ(APACHEⅡ评分)、血乳酸、C反应蛋白(CRP)水平及预后状况。结果:腹痛消失时间、血清淀粉酶恢复时间、胃肠功能恢复时间及ICU住院时间,2组比较差异均有统计学意义(P<0.05)。APACHEⅡ评分、血乳酸及CRP水平,2组比较差异有统计学意义(P<0.05)。总有效率观察组为93.33%,对照组为75.56%,2组比较差异有统计学意义(P<0.05)。多脏器功能障碍综合征及病死率,2组比较差异有统计学意义(P<0.05)。结论:在西医常规治疗的基础上给予大承气汤,可改善重症胰腺炎患者胃肠道功能,提高临床疗效。

Objective:To observe clinical effects of DaChengQi Tang in treating severe pancreatitis on the foundation of routine treatment of western medicine.Methods:All90patients were randomized into the observation group and the control group,45cases each group.Both groups accepted routine treatment,the control group received respiratory supportive treatment by using breathing machine based on routine treatment,the observation group took DaChengQi Tang on the basis of the therapy the control group accepted.Clinical effects,the disappearing time of abdominal pain,the recovery time of serum amylase,recovery time of gastrointestinal function,ICU hospitalization time of both groups were surveyed,the prognosis,the levels of CRP and lactic acid and APACHEⅡscales were assessed.Results:The difference had statistical meaning in the comparisons of the disappearing time of abdominal pain,the recovery time of serum amylase,recovery time of gastrointestinal function,ICU hospitalization time between both groups(P<0.05).The difference showed statistical meaning in the comparisons of the levels of CRP and lactic acid and APACHEⅡscales(P<0.05).Total effective rate of the observation group was93.33%,higher than75.56%of the control group,and the difference had statistical meaning(P<0.05).The difference had statistical meaning in the comparisons of multiple organ dysfunction syndrome(MODS)and mortality between both groups(P<0.05).Conclusion:DaChengQi Tang given to the patients with severe pancreatitis on the foundation of routine treatment of western medicine could improve gastrointestinal function of the patients and raise clinicaleffects.
