摘要
目的分析肌力平衡术治疗先天性马蹄内翻足(CCF)患儿的疗效。方法对14例(22足)6个月内及10例(16足)6个月~2岁先天性马蹄内翻足患儿进行软组织松解+胫前肌转位+矫形石膏外固定治疗,选取6个月内31例(48足)先天性马蹄内翻足患儿采用软组织松解+矫形石膏外固定治疗,分析患儿的临床疗效。结果 <6个月的患儿优良率为95.5%,明显高于6个月~2岁患儿的75.0%,差异具有统计学意义(P<0.05)。软组织松解+胫前肌转位+矫形石膏外固定治疗<6个月患儿优良率为95.5%,软组织松解+矫形石膏外固定治疗<6个月患儿优良率为91.7%,两种治疗优良率比较差异无统计学意义(P>0.05)。结论早期建立动态肌力平衡是治疗小年龄儿童先天性马蹄内翻足的有效方法,可矫正畸形,取得良好外观和功能,疗效满意。
Objective To analyze the curative effect of muscle balance operation in the treatment ofcongenital clubfoot(CCF)children younger than6months.Methods A total of14children(22feet)younger than6months and10children(16feet)older than6months and younger than2years with congenital clubfoot receivedsoft tissue release+tibialis anterior muscle transposition+orthopedic plaster external fixation for treatment.A total of31children(48feet)younger than6months with congenital clubfoot received soft tissue release+orthopedic plaster external fixation for treatment.Their clinical efficacy was analyzed.Results Children youngerthan6months had higher excellent-good rate as95.5%than75.0%in children older than6months and youngerthan2years,and the difference had statistical significance(P<0.05).Children younger than6months treated withsoft tissue release+tibialis anterior muscle transposition+orthopedic plaster external fixation had excellent-goodrate as95.5%,and children younger than6months treated with soft tissue release+orthopedic plaster externalfixation had excellent-good rate as91.7%.There was no statistically significant difference in excellent-good rateby two methods(P>0.05).Conclusion Early establishment of dynamic muscle balance is an effective method forthe treatment of congenital clubfoot in children of small age.It can correct deformity,achieve good appearance andfunction,and it has satisfactory curative effect.
