基质金属蛋白酶8与急性心肌梗死及冠脉病变的相关性研究 被引量:3

Study on the correlation between matrix metalloproteinase -8 and acute myocardial infarction and coronary artery disease

摘要 目的探讨基质金属蛋白酶-8(MMP-8)在急性心肌梗死及评价冠状动脉严重程度中的临床价值。方法连续选择2016年1月~2017年3月于中国石油天然气集团公司中心医院急性心肌梗死患者66例,对照组49例。比较两组血压、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、体质指数(BMI)等参数差异;比较两组MMP-8及肌钙蛋白I差异;根据冠脉造影结果将患者分为单支病变、双支病变和三支病变组,比较组间MMP-8、肌钙蛋白I、TG、TC等的差异。结果急性心肌梗死组收缩压、TG、TC、LDL-C显著高于对照组,HDL-C显著低于对照组(P<0.05),两组舒张压、BMI、肌酐、空腹血糖、无统计学差异(P>0.05)。急性心肌梗死组MMP-8(29.85±9.43)ng/ml,肌钙蛋白I(4.02±3.38)μg/L显著高于对照组(3.28±2.55)ng/ml和(0.08±0.067)μg/L(P<0.05),MMP-8与肌钙蛋白I呈显著正相关,相关系数r=0.739,P<0.01,三组间肌钙蛋白、TG、TC、空腹血糖等无统计学差异。结论 MMP-8水平可作为诊断急性心肌梗死及判断冠状动脉狭窄严重程度的有效辅助检测指标。 Objective To investigate the clinical value of matrix metalloproteinases-8(MMP-8)in acutemyocardial infarction(AMI)and coronary artery disease evaluation.Methods A total of66AMI patients were includedas AMI group.And44healthy subject were enrolled as control group.The differences of blood pressure,triglyceride,total cholesterol,high density lipoprotein cholesterol,low density lipoprotein cholesterol and body mass index werecompared between the two groups.The differences of MMP-8and troponin I between the two groups were compared.According to the results of coronary angiography,the patients were divided into single vessel disease,double-vesseldisease and three-vessel disease group.The differences of MMP-8,troponin I,triglyceride and cholesterol werecompared between the two groups.Results The levels of systolic blood pressure,triglycerides,total cholesterol,low density lipoprotein cholesterol in AMI group were significantly higher,and the level of high density lipoproteincholesterol was significantly lower compared with control group(P<0.05).There was no significant difference indiastolic blood pressure,body mass index,creatinine and fasting blood sugar between the two groups(P>0.05).InAMI group,MMP-8(29.85±9.43vs.3.28±2.55)ng/ml and troponin I(4.02±3.38vs.0.08±0.067)ug/l weresignificantly higher than control group(P<0.05).MMP-8was positively correlated with troponin I(r=0.739,P<0.01).There was no significant difference in troponin,triglyceride,cholesterol and fasting blood glucose among the threegroups.Conclusion MMP-8might be an effective biomarker in identification of AMI and evaluating the severity ofcoronary artery stenosis.
