胫骨高位截骨术治疗膝关节内侧间室骨关节炎的疗效

The effect of high tibial osteotomy in the treatment of osteoarthritis of the medial joint of the knee joint

目的探讨胫骨高位截骨术治疗膝关节内侧间室骨关节炎的临床疗效。方法选取2014年5月~2015年12月我院治疗的膝关节内侧间室骨关节炎患者65例,随机分为观察组33例及对照组32例。对照组采用腓骨截骨术治疗,观察组采用胫骨高位截骨术。对所有患者均随访至少12个月,并对其术前、3个月、6个月、12个月进行HSS膝关节评分,疼痛视觉模拟评分(VAS)及曼彻斯特大学关节炎指数(WOMAC),观察研究两组患者的治疗效果。观察两组的内侧间室高度、胫股角的变化。结果治疗后,两组患者的HSS评分较治疗前均显著升高(P<0.05);且观察组治疗后的HSS评分显著高于对照组(P<0.05)。与对照组比较,观察组术后3、6与12个月的VAS评分显著降低(P<0.05)。两组VAS评分分别于术后3、6与12个月逐渐降低(P<0.05)。观察组患者的WOMAC评分及FTA角度高于对照组,MPTA低于对照组(P<0.05)。结论胫骨高位截骨术治疗膝关节内侧间室骨关节炎的疗效显著,可明显缓解患者的症状。

Objective To observe the The effect of high tibial osteotomy in the treatment of osteoarthritis of themedial joint of the knee joint.Methods65patients with osteoarthritis of the knee joint in our hospital from May2014to December2015were randomly divided into the observation group(33cases)and the control group(32cases).The control group was treated with fibula osteotomy,and the observation group was treated with high tibial osteotomy.At least12months for all the patients were followed up,and the HSS knee score before surgery,3months,6months,12months,visual analogue scale(VAS)and University of Manchester arthritis index(WOMAC),observation on thetherapeutic effect of two groups.The height of the medial chamber and the angle of tibial stock were observed inthe two groups.Results After treatment,HSS scores of two groups were significantly higher than beforetreatment(P<0.05),and the observation group after treatment HSS score was significantly higher than the controlgroup(P<0.05).Two groups of patients at different times after treatment,VAS scores were lower than before surgery(P<0.05),the observation group after treatment VAS score was significantly lower than the control group(P<0.05).The WOMAC score and FTA angle of the observation group were higher than that of the control group,and MPTA waslower than that of the control group(P<0.05).Conclusion The effect of high tibial osteotomy in the treatment ofosteoarthritis of the medial joint of the knee joint is significant,which can obviously relieve the symptoms of the patients.
