急性淋巴细胞白血病T、B系及髓系相关抗原交叉表达情况研究

摘要 目的研究流式细胞术对急性淋巴细胞白血病(ALL)的T、B系特异性抗原及髓系相关抗原的交叉表达情况,指导ALL临床治疗和预后判断。方法应用流式细胞术对51例ALL患者进行细胞膜表面抗原及细胞质抗原标记,分析ALL分型结果。结果急性B淋巴细胞白血病(B-ALL)占78.4%,急性T淋巴细胞白血病(T-ALL)占21.6%。在ALL中,髓系抗原表达率为68.6%(35/51),在B-ALL和T-ALL中的表达率分别为72.5%(29/40)和54.5%(6/11),两个亚型比较差异无统计学意义(P>0.05)。CD13在CD34+B-ALL及CD34B-ALL中的表达分别为(52.2%、21.4%,P<0.05),CD13在CD34+T-ALL及0034丁-八1^中的表达分别为(42.9%、25.0%,尸<0.05)。0033在0034+:6-八〇^及0034B-ALL中的表达分别为46.2%、14.3%,P<0.05),CD33在CD34+T-ALL及CD34T-ALL中的表达分别为(42.9%、25.0%,P<0.05)。CD4在B-ALL中表达率为17.5%(7/40),4/7例伴CD13/CD33表达,其他T系抗原表达极少:1例表达CD7,同时伴CD13/CD33表达;1例表达CD2,同时伴CD13/CD33;2例表达CD3、3表达例CD5,均不伴CD13/CD33表达;1例共表达CD7和CDL结论对ALL细胞交叉表达特征的进一步研究,将有助于理解造血前体细胞和白血病干细胞的生长和分化,并对ALL的治疗及颈后提供帮助。 Objective To investigate the cross expression of T,B lineage specific antigen and myeloid M antigen in acute lym-phoblastic leukemia(ALL)by flow cytometry,and to guide the clinical treatment and prognosis of ALL.Methods The cell surface antigen and the cytoplasmic antigen were markered on51patients with ALL by flow cytometry,and the ALL typing results were analysed.Results B-ALL accounted for78.4%,T-ALL accounted for21.6%.The expression rate of myeloid antigen in ALL was68.6%(35/51),and the expression rates of myeloid antigen in B-ALL and T-ALL were72.5%(29/40)and54.5%(6/11)re-spectively.The difference between the two subtypes was no statistically significant(P^>0.05).The expression rate of CD13in CD34+B-ALL and CD34B-ALL was52.2%,21.4%(P<C〇.05)?and the expressionrate of CD13in CD34+T-ALL and CD34T-ALL was42.9%,25%respectively(P<C〇.05).The expression rate of CD33in CD34+B-ALL and CD34B-ALL was46.2%and14.3%respectively(P<C〇.05),and the expressionrate of CD33in CD34+T-ALL and CD34T-ALL was42.9%,25%respectively(P〈0.05).The expression rate of CD4in B-ALL was17.5%(7/40),and4/7cases were expressed with CD13/CD33.CD7was expressed in1cases and accompanied by CD13/CD33expression;CD2was expressed in1cases and accompanied by CD13/CD33;2cases expressed CD3and3expressed CD5without CD13/CD33expression;CD7and CD4were expressed in1cases.Conclusion Thefurther studies on the characteristics of cells cross expression of ALL will help to understand the growth and differentiation of hematopoietic precursor cells and leukemic stem cells?and offers to help for the treatment and prognosis of ALL.
