肝病相关自身抗体检测在自身免疫性肝病中的诊断价值研究

Study on diagnostic value of detection of liver-related autoantibodies in autoimmune liver diseases

目的探讨自身抗体检测在自身免疫性肝病诊断中的临床意义。方法选取2〇15年3月至2〇16年8月该院患者68例，按自身免疫性肝病、原发性胆汁性肝硬化、原发性硬化性胆管炎和重叠综合征分为4种类型疾病组，对照组为同期50例其他肝病患者；对5组患者进行抗核抗体及自身抗体检测，并对检测结果进行统计学分析。结果4种类型疾病组中抗核抗体阳性检出率均高于对照组，原发性硬化性胆管炎组阳性检出率（25.00%)低于另外3组（P<0.05);对照组及原发性硬化性胆管炎组各项检测指标均为阴性，4种类型疾病组中自身抗体阳性情况差异有统计学意义（P<0.05)。结论自身抗体检测在肝病诊断中，可有效将自身免疫性肝病与其他类型肝病相区别，且在一定程度上可对不同疾病类型进行鉴别，值得进一步推广使用。

Objective To investigate the clinical significance of autoantibody detection in the diagnosis of autoimmune liver dis-ease.Methods From March2015to August2016,68patients in the hospital were selected and were divided into4types of disease group by autoimmune liver disease,primary biliary cirrhosis,primary sclerosing cholangitis and overlap syndrome.The control group was50patients with other liver diseases in the same period.Antinuclear antibodies and autoantibodies were detected in the5groups,and the results were statistically analyzed.Results The positive rate of antinuclear antibodies in the4types of disease group was higher than that in the control group.The positive rate of primary sclerosing cholangitis group(25%)was lower than that of the other3groups(P<C0.05).The detection indexes of the control group and the primary sclerosing cholangitis group were all negative.The positive rate of autoantibody in the4types of disease group was statistically significant(P<C0.05).Conclusion Autoantibody detection can effectively differentiate autoimmune liver disease from other types of liver diseases,and can identify dif-ferent disease types to a certain extent,and is worthy of further promotion.
