颈髓损伤的常规MRI表现与DTI技术临床应用效果观察

目的探究颈髓损伤的常规MRI表现与DTI技术的临床应用效果。方法以来我院就医的46例疑似颈髓损伤患者(2014年12月~2016年12月)以及同期40例来我院行健康体检者(对照组)作为本次研究的观察对象,对46例疑似颈髓损伤患者及40例健康体检者均实施常规MRI与DTI技术检查,对比46例疑似患者及40例体检者的MRI与DTI技术检查结果、常规MRI表现与DTI技术的诊断准确率、敏感度、特异度。结果常规MRI表现与DTI技术诊断颈髓损伤的准确率为97.56%,将该检测结果和临床最终诊断结果进行比较可知,两者差异无统计学意义(P>0.05);常规MRI表现与DTI技术诊断颈髓损伤的敏感度为97.50%(39/40),特异度为66.67%(4/6);40例颈髓损伤患者的ADC值显著高于对照组,但FA值显著低于对照组(P<0.05),且急性颈髓压迫组、慢性颈髓压迫组、颈髓慢性压迫合并急性压迫组患者之间的ADC值和FA值差异有统计学意义(P<0.05)。结论对颈髓损伤患者采取常规MRI与DTI技术检查进行诊断切实可行,可为临床医师诊断颈髓损伤及其分型提供依据。

Objective To explore the routine MRI findings of cervical spinal cord injury and clinical effect of DTItechnique.Methods46patients with suspected cervical spinal cord injury cured in our hospital from December2014to December2016were selected,and40cases for physical examination in our hospital were selected as controlgroup.The46patients with suspected cervical spinal cord injury and the40cases of healthy person performed routineMRI and DTI technical examinations.The diagnostic accuracy,sensitivity and specificity of MRI and DTI techniques,routine MRI findings and DTI techniques of the two groups were explored and compared.Results The accuraterate of routine MRI and DTI diagnosis of cervical spinal cord injury was97.56%,the results of the test and the finaldiagnosis of the clinical comparison,there was no significant difference between the two(P>0.05).The sensitivityof conventional MRI scan and DTI technique in diagnosing cervical spinal cord injury was97.50%(39/40),and thespecificity was66.67%(4/6).The ADC value of40patients with cervical spinal cord injury was significantly higherthan that of the control group,but the FA value was significantly lower than that of the control group(P<0.05).Andthe ADC and FA values of acute cervical compression group,the chronic cervical spinal cord compression group,thecervical spinal cord chronic compression group with acute compression group were significantly different(P<0.05).Conclusion It is feasible to diagnose patients with cervical spinal cord injury by conventional MRI and DTItechniques.It can provide the basis for clinicians to diagnose cervical spinal cord injury and its typing.
