腹腔镜肠粘连松解术治疗粘连性肠梗阻的临床效果评价

Clinical effect evaluation of laparoscopic intestinal adhesions in the treatment of adhesive intestinal obstruction

目的分析腹腔镜肠粘连松解术治疗粘连性肠梗阻的临床效果。方法将2014年12月~2016年4月80例粘连性肠梗阻患者作研究对象并随机分组。开腹组实施传统开腹手术进行治疗,腹腔镜组则给予腹腔镜肠粘连松解术治疗。比较两组粘连性肠梗阻缓解率;手术完成所需要的时间、术中平均失血量、肠蠕动时间、术后住院时间;干预前后患者躯体情况、心理状况等生活质量指标;并发症发生率、梗阻复发率。结果腹腔镜组粘连性肠梗阻缓解率高于开腹组,P<0.05;腹腔镜组手术完成所需要的时间、术中平均失血量、肠蠕动时间、术后住院时间低于开腹组,P<0.05;干预前两组躯体情况、心理状况等生活质量指标相近,P>0.05;出院时腹腔镜组躯体情况、心理状况等生活质量指标优于开腹组,P<0.05。腹腔镜组并发症发生率、梗阻复发率显著低于开腹组,P<0.05。结论腹腔镜肠粘连松解术治疗粘连性肠梗阻的临床效果确切,可有效改善临床症状,松解粘连,手术操作时间短,术后肠蠕动快,并发症少,可降低再梗阻发生率,安全有效,值得推广。

Objective To analyze the clinical effect of laparos copic intestinal adhesions in the treatment of adhesiveintestinal obstruction.Methods80patients with adhesive intestinal obstruction from December2014to April2016were enrolled and randomly divided into two groups.Patients in laparotomy group were treated with traditionallaparotomy.Patients in laparoscopic group were treated with laparoscopic bowel acupuncture.The rate of remission ofintestinal obstruction,the time required for the operation to be completed,the average blood loss during operation,thetime of peristalsis,the time of hospitalization,the physical condition of the patients before and after the intervention,the quality of life,and obstruction recurrence rate of the two groups were compared.Results The remission rate ofadhesive intestinal obstruction in laparoscopic group was higher than that in laparotomy group(P<0.05).The timerequired for laparoscopic surgery,the average blood loss during operation,the time of bowel movement in laparoscopicgroup was less than those in laparotomy group(P<0.05).Before the intervention,the two groups were similar inbody condition,psychological status and other quality of life,P>0.05.When discharged,the quality of life,physicalcondition and psychological status in the laparoscopic group we re better than those in the laparotomy group,P<0.05.The complication rate and recurrence rate of laparoscopic group were significantly lower than those of laparotomygroup(P<0.05).Conclusion The clinical effect of laparoscopic intestinal adhesions in the treatment of adhesiveintestinal obstruction is definite.It can effectively improve the clinical symptoms,relax the adhesions.The operationtime is short,the postoperative bowel movement is quick,and the complication is less.It can reduce the incidence ofre-obstruction.It is safe and effective.It is worthy of promotion.
