硬膜外预置管用于经产妇分娩镇痛临床研究

Clinical study of epidural catheter for delivery analgesia in parturients

目的探讨硬膜外预置管用于经产妇分娩镇痛临床研究价值。方法选择足月待产经产妇270例作为研究对象,依据患者需求和分娩镇痛方式将产妇分为3组,A组选择自愿要求分娩镇痛无禁忌证的足月经产妇90例,B组同期硬膜外无禁忌证的足月经产妇90例,另设同期未行分娩镇痛的足月经产妇90例作为C组,记录3组产妇产程时间,观察3组产妇缩宫素的使用率、分娩结局、新生儿窒息率以及产后出血率。结果产程3组比较差异无统计学意义;会阴裂伤率A、B组比较差异无统计学意义,两组与C组比较差异有统计学意义(P<0.05);A、B组在缩宫素使用率和剖宫产率方面比较差异无统计学意义,两组与C组比较差异有统计学意义(P<0.05)。3组产妇在新生儿窒息率、产后出血率等方面比较差异无统计学意义。结论硬膜外预置管用于经产妇分娩镇痛安全性高,且满足产妇不同阶段镇痛需求,保证镇痛的及时性、有效性,具有重要的临床价值。

Objective To investigate the epidural tube for the clinical research of preset value.Methods The labor pain of parturient term to beproduced by maternal270cases as the research object,according to the needs of patients and the labor analgesia were divided into3groups,A groupvoluntarily requested analgesia without contraindications of foot menstruation90cases of pregnant women without contraindication in B group overthe same period of epidural foot menstruation of90pregnant women,not for another period of labor analgesia foot menstrual maternal90cases as Cgroup,recording3groups of maternal labor time,observe3groups of maternal oxytocin usage,pregnancy outcome,neonatal asphyxia rate and postpartumhemorrhage rate.Results The difference between the3Birth Process Group No statistical significance;rate of perineal laceration A,therewas no significant difference between B group,there was significant difference between the two group and C group(P<0.05);A,B group use rateand the rate of cesarean section in oxytocin had no significant difference,the difference between the two group and C group have statistical learningthe significance(P<0.05)of.3group in neonatal asphyxia,postpartum hemorrhage rate difference was not statistically significant.Conclusion Epiduralanalgesia for preset pipe by maternal high safety,and meet the needs of different stages of analgesia,analgesic to ensure timeliness,validity,has important clinical value.
