常规超声在乳腺病灶良恶性鉴别中的应用价值

The Value of Conventional Ultrasound in Differential Diagnosis of Benign and Malignant Breast Lesions

目的探究常规超声在乳腺病灶良恶性鉴别中的应用价值。方法选取80例乳腺肿块患者,采用常规超声对乳腺肿块患者进行检查,同时分析患者的声像图特征,与手术病理的检查结果进行对比。结果 80例乳腺肿块患者手术病理诊断有65例良性,15例恶性。患者经过常规超声检查,显示有58例良性患者,与手术病理诊断相比,符合率为89.23%,同时超声常规检查显示恶性肿块14例,与病理诊断相比,符合率为93.33%。对80例乳腺肿块患者进行超声声像图分析,良性患者的图像与恶性患者的图像有明显区别。声像图中,良性患者的图像显示有包膜,并且界限比较清晰,形态规则,内部的回声显示出均匀的状况,后方无衰减。恶性患者的声像图边界不太清楚,形态不规则,没有包膜,内部的回声显示不均匀状况,后方存在衰减。肿块在彩色多普勒下的特点,恶性肿瘤:RI值要高于0.7,纵横比高于1.0,PI值高于1.2。良性肿块:血流分级在0~1级以内,RI值要小于0.7,PI值要小于1.2。结论常规超声在乳腺病灶良恶性鉴别中有重要的应用价值,可以帮助鉴别诊断乳腺良性或者恶性病变,二维灰阶声像图是诊断乳腺肿块的重要依据。

Objective To explore the value of conventional ultrasound in benign and malignant breast lesions.Methods80patients with breast lumps were selected.Patients with breast masses were examined by routine ultrasound.The ultrasonographic features were analyzed and compared with the results of surgical pathology.Results80cases of breast lumps in patients with surgical pathology diagnosis of65cases of benign,15cases of malignant.Patients with conventional ultrasound examination showed58cases of benign patients,compared with the surgical pathology diagnosis,the coincidence rate was89.23%,while ultrasound routine examination showed14cases of malignant mass,compared with the pathological diagnosis,the coincidence rate was93.33%.80cases of breast lumps in patients with ultrasound sonographic analysis,benign patients with malignant images of patients with a significant difference.In the sonogram,the image of the benign patient shows a capsule,and the boundaries are clear,the morphological rules,the internal echo shows a uniform condition,and the rear has no attenuation.Malignant patient's sonogram boundary is not clear,irregular shape,no capsule,the internal echo shows uneven condition,the rear there is attenuation.Mass in the color Doppler under the characteristics of malignant tumors:RI value is higher than0.7,aspect ratio higher than1.0,PI value is higher than1.2.Benign mass:blood flow in the0to1level,RI value is less than0.7,PI value is less than1.2.Conclusion Conventional ultrasound in the benign and malignant breast lesions have important value in the identification,can help the differential diagnosis of benign or malignant breast lesions,two-dimensional gray-scale sonogram is an important basis for the diagnosis of breast mass.
