Effects of gross hematuria on the results in laboratory urine examination

摘要 目的探讨肉眼血尿对尿液多项化验检查结果的影响。方法回顾分析比较2014年1月至2015年12月间收治的80例有明显肉眼血尿的IgA肾病(IgAN)患儿、40例急性链球菌感染后肾小球肾炎(APSGN)患儿,分别在肉眼血尿期间、肉眼血尿消失后送检尿常规、尿蛋白定量、尿白蛋白定量、尿蛋白/肌酐、尿钙定量、尿钙/肌酐、肾早期损伤指标和尿蛋白电泳的变化情况。结果 IgAN和APSGN两组患儿在肉眼血尿期间的尿蛋白定量、尿蛋白/肌酐、尿钙定量、尿钙/肌酐和尿小分子蛋白百分比水平较肉眼血尿消失后明显升高,差异均有统计学意义(P均<0.05);而尿白蛋白定量、肾早期损伤各指标(如微量白蛋白、α1微球蛋白水平、N-乙酰-β-D-葡萄糖苷酶)、血钙/肌酐、尿白蛋白百分比水平与肉眼血尿消失后比较,差异均无统计学意义(P均>0.05)。肉眼血尿期间,IgAN和APSGN两组患儿的尿蛋白定量水平均高于尿白蛋白定量水平(P<0.001);而肉眼血尿消失后,两组患儿的尿蛋白定量与尿白蛋白定量水平之间的差异均无统计学意义(P>0.05)。结论肉眼血尿时尿蛋白、尿钙定量结果均偏高,而尿白蛋白定量则不受影响。 Objectives To explore the effects of gross hematuria on the results of several parmeters in laboratoryurine examination.Methods Eighty(80)children with IgA nephropathy and40cases with acute post-streptococcalglomerulonephritis hospitalized in our hospital from January2014to December2015were recruited.The ratio of urinarycalcium and protein to creatinine,quantitative test of24h urinary calcium and protein,quantitative test of24h urinary albumin,α1-microglobulinuria,microalbuminuria and urine protein electrophoresis were tested during and after the gross hematuria,respectively.Results The ratio of urinary calcium and protein to creatinine,quantitative test of24h urinary calcium andprotein were much higher in the duration of gross hematuria as compared to those after the duration of gross hematuria,whileα1-microglobulinuria,microalbuminuria and quantitative test of24h urinary albumin showed no difference between the two periods.Conclusions Gross hematuria could increase the level of urinary calcium and protein,while quantitative test of24h urinaryalbremin is not affected.
