集束化护理干预在皮瓣移植修复巨大头皮合并颅骨缺损中的应用研究

Application of cluster nursing intervention in reconstruction of giant scalp combined with skull defect by flap transplantation

目的探究集束化护理干预在皮瓣移植修复巨大头皮合并颅骨缺损中的护理效果。方法取2015年12月~2016年12月医院收治的皮瓣移植修复巨大头皮合并颅骨缺损患者120例,随机数字法分为对照组(n=60)和观察组(n=60)。对照组采用常规方法护理,观察组在对照组基础上联合集束化护理干预,比较2组皮瓣移植成活率、并发症发生概率(颅内感染、出血、癫痫)、护理满意度等指标。结果观察组患者的皮瓣移植成活率(96.67%)、并发症发生概率(0.00%)均优于对照组患者的皮瓣移植成活率(75.00%)、并发症发生概率(6.67%)(P<0.05)。2组护理前VAS评分比较差异无统计学意义(P>0.05);观察组护理后1周、2周3周VSA评分,低于对照组(P<0.05)。结论皮瓣移植修复巨大头皮联合颅骨双重缺损患者在常规护理基础上联合集束化护理干预效果理想,值得推广应用。

Objective To explore the nursing effect of bundle nursing intervention in the reconstruction of giant scalp and skull defect with flap transplantation.Methods From December2015to December2016,120patients with giant scalp combined with skull defect were treated with skin flap transplantation in our hospital.The patients were randomly divided into control group(n=60)and observation group(n=60).The control group received conventional nursing,the observation group in the control group based on the collection of bundle nursing intervention,compared2groups of flap survival rate,the probability of occurrence of transplant complications(bleeding,intracranial infection,epilepsy),nursing satisfaction index.Results The observation group of patients with skin flap transplantation survival rate(96.67%),the probability of occurrence of complications(0%)were better than the control group of patients with skin flap transplantation survival rate(75%),the probability of occurrence of complications(6.67%),the difference was statistically significant between the two groups of data,P<0.05.There was no significant difference between the2groups in the VAS score before nursing(P>0.05).The scores of VSA in the observation group after1,2and3weeks were lower than that of the control group(P<0.05).Conclusion Scalp flap transplantation in patients with skull defect of huge double of the effect of nursing intervention on the basis of routine nursing care in the ideal collection beam,it is worthy of popularization and application.
