不同术式子宫手术对生育期妇女卵巢功能的影响研究 被引量:1

摘要 目的探究不同术式子宫手术对生育期妇女卵巢功能的影响。方法 120例子宫肌瘤患者,按照手术方式不同分为全子宫切除组(采用全子宫切除术治疗,45例)、次全子宫切除组(采用次全子宫切除术治疗,40例)、肌瘤剔除组(采用肌瘤剔除术治疗,35例)。比较手术前、术后3个月、术后6个月三组患者雌二醇水平及更年期潮热症状情况。结果肌瘤剔除组患者手术前后黄体生成素水平、卵泡刺激素水平、雌二醇水平比较差异无统计学意义(P>0.05)。与手术前及肌瘤剔除组比较,全子宫切除组、次全子宫切除组30~44岁患者术后3个月雌二醇水平显著下降,卵泡刺激素显著上升(P<0.05);全子宫切除组和次全子宫切除组45~50岁患者术后3个月激素水平变化不明显,差异无统计学意义(P>0.05);术后6个月,全子宫切除组和次全子宫切除组患者血清中雌二醇水平明显下降,卵泡刺激素明显上升(P<0.05)。肌瘤剔除组患者潮热发生率(8.6%)显著低于全子宫切除组(42.2%)、次全子宫切除组(40.0%)(P<0.05)。结论对45~50岁妇女实施全子宫切除术以及次全子宫切除术,会明显影响患者卵巢功能,卵巢会提前衰退。
