摘要
目的:通过观察中药龙胆泻肝汤加减联合点阵激光治疗白癜风患者的皮损治疗前后变化,证实其外观产生色素情况、毛囊隆突区储存的黑素干细胞数量和有毛皮肤白斑皮损出现毛囊复色变化情况相一致。方法:采用单株毛囊提取仪环钻法皮肤切除术结合逆转录聚合酶链式反应技术检测64例白癜风皮损组织中黑素细胞谱系特异性标记基因(Dct,Try,ACTB)的表达。结果:用环钻皮肤切削术获取1mg的组织标本,经Trizol法抽测到的总RNA,能在正常皮肤组织中检测到多巴色素异构酶(Dct)、酪氨酸酶(Tyr)和管家基因β-肌动蛋白(ACTB)3种基因mRNA的表达其表达模式为:Dct+Tyr-ACTB+,Dct-Tyr+ACTB+,Dct+Tyr+ACTB+;中药龙胆泻肝汤加减联合点阵激光组毛囊覆色明显优于点阵激光治疗白癜风组。结论:白癜风白斑皮损出现毛囊复色与毛囊隆突区储存的黑素干细胞(MP)数量变化呈正相关,说明白癜风白斑皮损出现毛囊复色是一种有价值的预测白癜风疗效的准确方法;并进一步从基础研究上证明中药干预联合点阵激光治疗白癜风疗效更好。
Objective:To observe the changes of skin lesions of vitiligo patients between before and after treated with the Longdan Xiegan decoction plus lattice laser,which was confirmed that the pigment and the amount of melanocyte stem cells in the Hair follicle ridge area have the same trend with the variation of PR after hair skin vitiligo skin lesion.Methods:Expression of the specific marker gene(Dct,Try,Actb)of melanocytes in64cases of vitiligo lesions was detected by RT-PCR.Results:Dct,Tyr and ACTB were detected in normal skin tissues.The expression pattern is Dct+Tyr-ACTB+,Dct-Tyr+ACTB+,Dct+Tyr+ACTB+.Hair follicle coating in the Longdan Xiegan decoction plus lattice laser group was superior to the lattice laser treatment of vitiligo group.Conclusion:There was a positive correlation between the change of hair follicle and the amount of MP stored in hair follicle,suggested that vitiligo white skin lesions appear hair follicle complex color is a valuable way to predict the efficacy of vitiligo,which prove that TCM medicine intervention plus lattice laser on vitiligo was better.
