摘要
终末期肾病患者只能依靠维持性治疗(包括肾透析和肾移植抗排异)生存,治疗费用高昂。应优先选择直接与间接医疗费用低、治疗效果优、社会回归程度高的经济适宜技术,以减轻患者和社会负担。研究发现,腹膜透析具备上述技术优势,但由于医护人员激励不足而不被临床优先选择。应通过三医联动改革的方式,规范临床操作的技术路径,对诊疗服务项目合理定价,在绩效考核中激励医护人员应用适宜透析技术,并通过医保支付方式优化予以制度保障,联动破解"因病致贫"。
The maintenance treatment for End-Stage Renal Disease,including dialysis and kidney transplant anti-rejection therapy,costs high.To reduce the economic burden on patient and society,the therapy method with the advantage of low direct and indirect costs,good treatment effects,and high returning to society should be recommended as fi rst choice.Peritoneal dialysis is the one with all mentioned advantage.However,because of low payment and award,doctors and nurses didn’t take PD as fi rst.Trough“Tripartite reform in health system”,including standardizing the technical path in clinical operation,pricing medical service reasonably,incenting doctors and nurses to provide PD fi rst via KPI,and optimizing the health insurance system,PD will be popularized.Via the“Tripartite reform in health system”for PD treatment,poverty caused by disease will decrease.
