超声诊断对长期服用抗精神分裂药物引起肝胆损害患者的观察研究

The observation study of ultrasonic diagnosison the liver and gallbladder damage caused by long-term use of anti-schizophrenia drugs

目的通过腹部超声观察分析长期服用抗精神分裂药物引起的肝胆损害特点及趋势。方法选取2013年8月～2016年4月在本院就诊并符合标准的精神分裂症患者为研究对象，其中服抗精神分裂药时间为3～5年者为观察组I，服药时间>5年者为观察组II，另外随机选定年龄>30岁且从未服用过抗精神分裂药者90名作为对照组。记录并分析受检者的肝胆超声改变。结果观察组II患者胆囊结石、慢性胆囊炎的发病率明显高于其余两组（P＜0.05），观察组I和对照组之间差异无统计学意义，观察组II患者单纯性脂肪肝的发病率明显高于对照组（P＜0.05），观察组II和观察组I之间差异无统计学意义，观察组I和对照组之间差异无统计学意义，3组患者肝囊肿、肝海绵状血管瘤的发病率的差异无统计学意义。结论长期服用抗精神分裂药可增加单纯性脂肪肝、胆囊结石、慢性胆囊炎等疾病的发病风险。

Objective To observe and analyze the characteristics and trends of the liver and gallbladder damage caused by long-term use ofanti schizophrenia drugs through the observation of the abdominal ultrasound.Methods From August2013to April2016,the patients diagnosed asschizophrenia and met criterions were enrolled in our hospital.The observation group I included patients taking anti schizophrenia drug for3to5years.The observation group II included patients taking anti schizophrenia drug more than5years.In addition,the control group recruited healthyperson who were over30years old and had never taken anti-schizophrenia drug.The change in ultrasound of liver and gallbladder were recorded andanalyzed.Results The rate of cholecystolithiasis,chronic cholecystitis in observation group II were higher than those in the other two groups(P<0.05),while there had no significant difference between observation group I and control group.The rate of pure fatty liver in observation group II werehigher than those incontrol group(P<0.05),while there had no significant difference between observation group I and observation group II,andthere had no significant difference between observation group I and control group.There had no significant difference among three groups regardingas the rate of hepatic cyst and hepatic cavernous hemangioma.Conclusion Long-term use of anti-schizophrenia drugs could increase the riskofcholecystolithiasis,chronic cholecystitis and pure fatty liver.
