胰腺损害患儿的病例分析

摘要 目的:研究分析危重症患儿合并胰腺损害的临床特点。方法:PICU收治的危重组患者,就年龄、性别、民族、血常规、血钙、血糖、LDH、肾功能、CRP、血淀粉酶及胰腺影像学等资料进行分析。结果:符合纳入条件的患儿102例,受累组23例(22.5%),非受累组79例(77.5%)。在LDH、CRP两个指标方面,受累组高于非受累组(P均<0.05)。两组血淀粉酶于入院后72 h差异有统计学意义(P<0.05)。结论:危重症患儿常合并胰腺损害,加重病情,应高度重视,及时检出及治疗。早期及动态监测血淀粉酶、LDH、CRP有助于诊断继发性胰腺损害,及时治疗,降低病死率。 Objective:To investigate the clinical characteristics of critically ill children complicated with pancreatic damage.Methods:The data of age,sex,ethnicity,blood routine,serum calcium,blood glucose,LDH,renal function,CRP,amylase andpancreatic imaging were analyzed in critically ill patients in PICU.Results:Of the102eligible children,23cases wereinvolved(22.5%),and79cases(77.5%)were not involved,in the two indicators of LDH and CRP,the involvement group was higherthan the non intervention group(P all<0.05),there was significant difference between the two groups in blood amylase at72h afteradmission(P<0.05).Conclusion:Children with critical illness often suffer from pancreatic damage and aggravate the disease.Theyshould be paid great attention to and detected and treated in time.Early and dynamic monitoring of serum amylase,LDH and CRPmay be helpful in diagnosing secondary pancreatic damage,prompt treatment and reducing mortality.
