Our community center explored the integrated management mode of diabetes in community hospitals,and implementedthe integrated management of diabetes prevention-treatment-rehabilitation,ward-outpatient care-community management,whichimproved the management rate and blood sugar control rate of diabetic patients in the community,and reduced the incidence andcomplications of diabetes mellitus.
