库区奉节县心因性勃起功能障碍的心理致病因素分析

Analysis on psychological pathogenic factors of psychogenic erectile dysfunction in reservoir area of Fengjie county

目的分析库区奉节县心因性勃起功能障碍(ED)的心理致病因素。方法 76例于库区奉节县泌尿外科就诊的心因性ED患者为病例组,152例非心因性ED患者为对照组,采用中国科学院心理研究所修订的明尼苏达多项人格量表(MMPI)-2(1989年版)对研究对象进行测验,通过单因素和多因素分析鉴定库区奉节县心因性ED的心理致病因素。结果单因素分析结果显示,两组癔症性格倾向、人格变异倾向、抑郁性格倾向、强迫性格倾向、内向性格倾向和疑病性格倾向患者例数比较差异具有统计学意义(χ~2=11.335、4.746、8.964、8.526、8.797、6.806,P<0.05),而躁狂性格倾向、分裂性格倾向和偏执性格倾向例数比较差异无统计学意义(P>0.05)。多因素分析结果显示,癔症性格倾向、抑郁性格倾向、强迫性格倾向和疑病性格倾向是库区奉节县心因性ED的心理致病因素(Wald-χ~2=6.081、6.754、12.684、11.254,P<0.05),其危险度排序为(调整OR值):强迫性格倾向>疑病性格倾向>抑郁性格倾向>癔症性格倾向(OR=5.914、5.247、4.738、4.173)。且癔症性格倾向、人格变异倾向、抑郁性格倾向、强迫性格倾向、内向性格倾向和疑病性格倾向相互之间均不存在交互作用。结论癔症性格倾向、抑郁性格倾向、强迫性格倾向和疑病性格倾向是库区奉节县心因性ED的心理致病因素。在心因性ED的临床治疗过程中,应注意改善和治疗ED患者的心理致病因素,通过详细的病史询问及心理沟通,透彻了解患者的性心理活动并予以纠正,并结合女方的配合完成最终的治疗。

Objective To analyze the psychological pathogenic factors of psychogenic erectiledysfunction(ED)in reservoir area of Fengjie county.Methods There were76psychogenic ED patients in urologysurgery department of Fengjie county as case group,and152non-psychogenic ED patients as control group.Thesubjects were tested by minnesota multiple personality scale(MMPI)-2(1989edition)revised by the Instituteof Psychology,Chinese Academy of Sciences,and the psychopathogenic factors of Fengjie County were analyzedby single factor and multivariate analysis.Results Single factor analysis showed that there were statisticallysignificant difference in cases of hysteria personality tendency,personality variation tendency,depressionpersonality tendency,forced personality tendency,introverted personality tendency and hypocritical personalitytendency(χ2=11.335,4.746,8.964,8.526,8.797,6.806,P<0.05),and there was no statistically significantdifference in cases of manic personality tendency,split personality tendency and paranoid personality tendency(P>0.05).Multiple analysis showed that hysteria personality tendency,depression personality tendency,forcedpersonality tendency and hypocritical personality tendency were psychological pathogenic factors of psychogenicED in reservoir area of Fengjie county(Wald-χ2=6.081,6.754,12.684,11.254,P<0.05),with risk ranking(adjusted OR value)as forced personality tendency>hypocritical personality tendency>depression personalitytendency>hysteria personality tendency(OR=5.914,5.247,4.738,4.173).There was no interaction in hysteriapersonality tendency,personality variation tendency,depression personality tendency,forced personality tendency,introverted personality tendency and hypocritical personality tendency.Conclusion Hysteria personalitytendency,depression personality tendency,forced personality tendency and hypocritical personality tendency werepsychological pathogenic factors of psychogenic ED in reservoir area of Fengjie county.In the clinical treatment ofpsychogenic ED,attention should be payed to the improvement and treatment of psychological factors in patientswith erectile dysfunction ED.The sexual psychological activities of ED patients should be understand thoroughlyand corrected,and combined with the woman’s cooperation to complete the final treatment.
